富血小板纤维蛋白上清对大鼠特应性皮炎改善效果的机制研究

摘　　要：目的明确富血小板纤维蛋白上清(PRF-s)对大鼠特应性皮炎的治疗作用及可能存在的机制。方法32只SD大鼠采用随机数字表法分为对照组、模型组、冰黄肤乐软膏组(软膏组)和PRF-s组,每组8只。大鼠背部剃毛后,除对照组,其余3组均建立特应性皮炎模型。应用未抗凝的大鼠尾静脉血制备PRF-s。软膏组和PRF-s组在造模完成后第2天给药,连续治疗10 d。病理验证特应性皮炎模型组皮损变化。3组均于治疗前后进行皮损评分,免疫组织化学染色检测病变部位白细胞介素(IL)-4、干扰素-γ(IFN-γ)的表达情况,Western blot检测基质金属蛋白酶(MMP)-9和磷酸化p38丝裂原活化蛋白激酶(p-p38 MAPK)的变化。结果造模后大鼠出现搔抓、翻滚等表现,且皮损部位病理显示皮肤角化异常,真皮水肿,可见巨噬细胞及中性粒细胞浸润。软膏组和PRF-s组治疗后皮损评分分别为(1.80±0.36)分和(1.80±0.27)分低于模型组(4.90±0.44)分(P<0.01)。与模型组比较,软膏组和PRF-s组治疗后病变部位IL-4、MMP-9和p-p38 MAPK表达下调,IFN-γ表达升高(P<0.05)。结论PRF-s可能通过信号分子MMP-9和p38 MAPK来调控特应性皮炎局部免疫失衡和炎性浸润,从而改善症状。Objective To clarify the therapeutic effect of platelet rich fibrin supernatant(PRF-s)on atopic dermatitis(AD)in rats and its possible mechanism.Methods In total,32 SD rats were divided into control group(n=8),model group(n=8),Binghuangfule ointment group(ointment group,n=8),and PRF-s group(n=8)according to random number table method.After shaving the back of rats,AD models were established in all three groups except the control group.PRF-s were prepared from non anticoagulated venous blood from the rat tail.The ointment group and PRF-s group were administered on the second day after the completion of modeling and treated continuously for 10 d.Pathology was employed to verify changes in skin lesions in AD models.All three groups underwent skin lesion scoring before and after treatment,and immunohistochemical staining was used to detect the expression of interleukin(IL-4)and interferon-γ(IFN-γ)in the lesion site.Western blot was used to detect the changes of matrix metalloproteinase(MMP)-9 and p-p38 mitogen activated protein kinase(p-p38 MAPK).Results After modeling,the rats experienced scratching,rolling,and the pathology of the skin lesions showed abnormal keratinization,dermal edema,macrophages and neutrophils infiltration.The skin lesion scores in ointment group and PRF-s group were(1.80±0.36)and(1.80±0.27)respectively,which were lower than that(4.90±0.44)in the model group(P<0.01).Compared with the model group,the ointment group and PRF-s group showed down-regulation of IL-4,MMP-9,and p-p38 MAPK expression in the lesion site after treatment,and up-regulation of IFN-γexpression(P<0.05).Conclusion PRF-s may regulate local immune imbalance and inflammatory infiltration through signal molecules MMP-9 and p38MAPK,so as to improve the symptoms of AD.
