摘  要:目的评估糖尿病视网膜病变(DR)患病现状及利用人工智能(AI)在DR早期筛查的适用性。方法收集就诊的糖尿病患者548例,利用人工智能糖尿病视网膜病变筛查系统(AI-DRS)对眼底照相进行判读并由专家进行复核。收集患者一般状况、病程、血糖控制情况及治疗方案等基本情况和其他糖尿病并发症相关数据。分析DR患病状况,视力损害状况及相关危险因素,AI-DRS筛查DR的阳性率、符合率。结果糖尿病患者中DR的患病率为47.3%,年龄越大,病程越长越容易出现DR。糖尿病患者中至少一眼视力损害占11.3%,空腹血糖、餐后血糖控制不佳或合并高血压会增加糖尿病患者视力损害的风险。11.6%DR出现视力损害,均未进行治疗。8.5%DR患者因眼底检查而发现有糖尿病转内分泌科就诊。出现糖尿病神经系统或多系统并发症时更容易出现视力损害。结论利用人工智能DR筛查的技术以二级医院眼科为筛查基地是目前开展DR早期筛查的有效途径。Objective To evaluate the status of prevention and treatment of diabetic retinopathy(DR)in primary hospitals with the applicability of artificial intelligence(AI)system in DR screening.Methods The data and fundus image of patients with diabetes mellitus were collected.Artificial Intelligence Diabetic Retinopathy system(AI-DRS)were applied for DR screening and reviewed by ophthalmologists from Ningxia eye hospital.Besides,the general condition,course of disease,blood glucose control,treatment plan and diabetic complications were collected.The prevalence of DR,visual impairment status and related risk factors of DR were analyzed.The positive rate and coincidence rate between AI and ophthalmologists were evaluated.Results The prevalence of DR was 47.3%among the patients with diabetes mellitus.The older ones with the longer the course of disease,was prone to suffer DR.The vision impairment in at least one eye with diabetes accounts for 11.3%.Fasting blood glucose,poor control of postprandial blood glucose or hypertension may increase the risk of visual impairment in diabetics.11.6%of DR had visual impairment without any treatment.There were 8.5%DR patients were first identified diabetes after DR screening,and transferred to the Department of Endocrinology.Visual impairment was more common on whom with multiple system diabetic complications.Conclusion It is an effective way to carry out early DR screening by means of artificial intelligence in the ophthalmology department of secondary hospital.
