地佐辛在剖宫产术中应用及其对血清疼痛介质表达和机体免疫应答的影响

摘　　要：目的探讨地佐辛在剖宫产术中应用及其对血清疼痛介质表达和机体免疫应答的影响。方法选择104例剖宫产者,将其分为观察组(n=52)、对照组(n=52)。2组均使用腰硬联合麻醉,观察组在胎儿取出后给予地佐辛静脉注射,对照组不使用。比较2组视觉模拟疼痛评分(VAS)、血清疼痛介质、机体免疫应答变化及术后不良反应。结果观察组在术后T1(6 h)、T2(12 h)、T3(24 h)、T4(48 h)的VAS评分,前列腺素E_(2)(PGE_(2))、P物质(SP)、β-内啡肽(β-EP)的水平均低于对照组,免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)的水平均高于对照组(P<0.05);观察组和对照组术后不良反应总发生率分别9.62%、15.38%,2组比较差异无统计学意义(P>0.05)。结论地佐辛在剖宫产术麻醉中具有降低血清疼痛介质表达和保护机体免疫应答的作用,值得临床推广。Objective To explore the application of dezocine in cesarean section and its effect on serum pain mediators and immune response.Methods 104 pregnant women undergoing cesarean section were randomly divided into the observation group(n=52)and the control group(n=52).Both groups were treated with the spinal-epidural anesthesia.The observation group was given intravenous injection of dezocine after fetal delivery,while the control group was not used.The visual analogue pain score(VAS),serum pain mediator,changes of immune response and postoperative adverse reactions were compared between the two groups.Results The VAS scores,prostaglandin E2(PGE2),substance P(SP),β-endorphin in the observation group were lower than those in the control group,while the indicator levels of immunoglobulin A(IgA),immunoglobulin G(IgG),and immunoglobulin M(IgM)were higher than those in the control group(P<0.05).The total incidence of adverse reactions in the observation group and the control group was 9.62%and 15.38%,there was no significant difference(P>0.05).Conclusion Dezocine can reduce the expression of serum pain mediators and protect the immune response of pregnant women undergoing cesarean section anesthesia.It is worthy of clinical promotion.
