二维超声心动图评估主动脉瓣反流患者左心室构型改变的价值

摘　　要：目的 探讨二维超声心动图评估主动脉瓣反流患者左心室构型改变的价值。方法 选取2018年3月至2021年7月在我院就诊的主动脉瓣反流患者389例,以主动脉瓣反流程度分为轻度反流组(300例)、中度反流组(80例)、重度反流组(9例),同时选取100例健康体检者作为对照组。4组研究对象均经二维超声动态心动图检查,比较二维超声心动图检测的结果,包括收缩末期左心室长径(Ls)、舒张末期左心室长径(Ld)、收缩末期、舒张末期球形指数(SIs、SId),将垂直于左心室的长径三等分的三个横径(收缩期为d1、d2、d3,舒张期为D1、D2、D3),收缩期、舒张期左心室的侧壁横径(LSs、LSd),(d1+d2+d3)/3Ls和(D1+D2+D3)/3Ld及与左心室横径垂直的前后径(APs、APd)。结果 4组研究对象的年龄、性别、体重、身高、心率、体表面积比较,差异无统计学意义(均P>0.05)。轻度反流组患者左心室构型的长轴参数与对照组比较,差异无统计学意义(P>0.05);中度反流组患者的Ls、Ld、SIs、D1、D2、D3、d1、d2、d3、(D1+D2+D3)/3Ld均明显大于轻度组和对照组,而SId、(d1+d2+d3)/3Ls明显小于轻度组和对照组,差异均有统计学意义(均P<0.05);重度反流组患者的Ls、Ld、SIs、D1、D2、D3、d1、d2、d3、(D1+D2+D3)/3Ld明显大于轻度反流组、对照组和中度反流组,SId、(d1+d2+d3)/3Ls明显小于轻度反流组、对照组和中度反流组,差异均有统计学意义(均P<0.05)。对照组与轻度反流组左室构型的短轴参数比较,差异无统计学意义(P>0.05);中度反流组的LSs、LSd、APs、APd明显大于轻度反流组和对照组,LSs/APs明显小于轻度反流组和对照组,差异均有统计学意义(均P<0.05);重度反流组的LSs、LSd、APs、APd明显大于中度反流组、轻度反流组和对照组,LSs/APs明显小于轻度反流组和对照组,差异均有统计学意义(均P<0.05)。重度反流组与中度反流组患者的LSs/APs比较,差异无统计学意义(P>Objective To analyze the value of 2-D echocardiography in left ventricular configuration changes in patients with aortic regurgitation.Methods A total of 389 patients with aortic regurgitation in our hospital from March 2018 to July 2021 were selected and divided into mild regurgitation group(300 cases),moderate regurgitation group(80 cases)and severe regurgitation group(9 cases)according to the degree of aortic regurgitation,and 100 healthy people were selected as the control group during the same period.The results of 2D echocardiography were compared,including left ventricular length(Ls)in end-systolic patients,left ventricular length(Ld)in end-diastolic patients,end-systolic and end-diastolic sphericity index(SIs,SId),and left ventricular length(Ls)in end-diastolic patients.Three transverse meridians perpendicular to the long diameter of the left ventricle(d1,d2,d3 in systole and D1,D2,D3 in diastole),the transverse meridians of the left ventricle in systole and diastole(LSs,LSd),(d1+d2+d3)/3Ls and(D1+D2+D3)/3Ld and anteroposterior diameters perpendicular to the left ventricular transverse meridian(APs,APd).Results There were no differences in age,gender,weight,height,heart rate and body surface area among the four groups(P>0.05).There was no difference in the long axis parameters of left ventricular configuration between mild group and control group(all P>0.05).The Ls,Ld,SIs,D1,D2,D3,d1,d2,d3 and(D1+D2+D3)/3Ld of moderate group were significantly higher than those of mild group and control group,while SId and(d1+d2+d3)/3Ls were significantly lower than those of mild group and control group(all P<0.05).The Ls,Ld,SIs,D1,D2,D3,(D1+D2+D3)/3Ld of patients in severe regurgitation group were significantly higher than those in mild regurgitation group,control group and moderate regurgitation group(P<0.05).The SId and(d1+d2+d3)/3Ls of severe regurgitation group were significantly lower than those of mild regurgitation group,control group and moderate regurgitation group(P<0.05).There was no difference between the con
