足三阴经筋理论指导小针刀松解对股骨头坏死患者髋关节功能的影响

摘　　要：目的:观察足三阴经筋理论指导小针刀松解股内收肌群对股骨头坏死患者髋关节功能的影响。方法:选择存在股内收肌群挛缩的股骨头坏死患者80例,随机分为治疗组和对照组各40例。治疗组采用小针刀松解股内收肌群的结筋病灶点联合经筋推拿手法治疗,对照组仅采用经筋推拿手法治疗。比较两组患者治疗前、治疗后3个月、治疗后6个月的髋关节外展活动度、视觉模拟评分法(VAS)评分、Harris髋关节功能评分和国际髋关节结果工具(iHOT-12)评分。结果:治疗后各随访时间点两组患者的各疗效指标水平均优于治疗前(P<0.05),且治疗组各疗效指标水平均优于对照组(P<0.05)。结论:对于存在股内收肌群挛缩的股骨头坏死患者,足三阴经筋理论指导小针刀松解股内收肌群可缓解股内侧疼痛,改善髋关节外展活动度,有助于提高临床保髋治疗的有效率。Objective:To observe the effect of acupotomy release of adductor femoris guided by the theory of three yin sinews and meridians of foot on hip joint function in patients with femoral head necrosis.Methods:A total of 80 patients with femoral head necrosis and contracture of adductor femoris were divided into treatment group and control group,with 40 cases in each group.The control group was treated with sinew and meridian massage,and the treatment group was treated with acupotomy to release the sinew knots in adductor femoris on the basis of the therapy given to the control group.Before treatment,3 months after treatment,and 6 months after treatment,the hip abduction range of motion,visual analogue scale(VAS)score,Harris hip joint function score and International Hip Outcome Tool(iHOT-12)score were compared between the two groups.Results:The levels of the outcome measurements in the two groups at each follow-up point after treatment were better than those before treatment(P<0.05),and the levels of the outcome measurements in the treatment group at each follow-up point after treatment were better than those in the control group(P<0.05).Conclusions:For patients with femoral head necrosis and contracture of adductor femoris,acupotomy release of adductor femoris guided by the theory of three yin sinews and meridians of foot can relieve medial femoral pain,improve hip abduction range of motion,which is helpful to increase the effective rate of hip preservation therapy.
