摘　　要：目的分析血清5-羟色胺(5-HT)水平和系统性硬化症(SSc)患者临床表现及心脏核磁的相关性,探讨其临床意义。方法前瞻性连续纳入2021年12月到2022年8月在北京协和医院风湿免疫科门诊就诊的SSc患者,对其临床特征、实验室检查和心脏核磁进行分析,同时进行5-HT检测。使用Spearman秩相关系数进行相关性分析,筛选出有统计学意义指标作为自变量进行线性回归分析。结果本研究共纳入30例SSc患者,女性27例(90%),SSc发病年龄(46.0±2.4)岁,其中16例(53.3%)患者为弥漫性皮肤型系统性硬化症(dcSSc)。相关分析示男性与5-HT呈正相关(r=0.520,P=0.003)。SSc患者心脏核磁的心肌固有T1值为(1302.4±46.5)ms,与血清5-HT呈负相关(r=-0.405,P=0.026)。将性别、心肌固有T1值纳入模型1,性别、心肌固有T1值、反流性食管炎纳入模型2进行多重线性回归分析:SSc患者性别与5-HT模型1(β:671.399,95%CI:534.430,808.368,P<0.001),模型2(β:670.398,95%CI:534.598,806.199,P<0.001)均显著相关。心肌固有T1值与5-HT模型1(β:-1.510,95%CI:-2.238,-0.782,P<0.001),模型2(β:-1.474,95%CI:-2.198,-0.750,P<0.001)均显著相关。结论5-HT与SSc早期心肌病变可能有关,两者的相关性有待进一步扩大样本量的研究证实。Objective To investigate the association between serum 5-hydroxytryptamine(5-HT)and clinical manifestations and cardiac magnetic resonance imaging in patients with systemic sclerosis(SSc).Methods Patients with SSc were prospectively and consecutively registered in the outpatient of the Department of Rheumatology and Clinical Immunology in Peking Union Medical College Hospital from December 2021 to August 2022.The clinical features,laboratory tests,and cardiac magnetic resonance imaging findings were analyzed,and 5-HT levels were also tested.Spearman’s rank correlations and linear regression analyses were conducted to determine the association between 5-HT and other variables.Results A total of 30 patients with SSc were recruited in the study,including 27 females(90%),and the age of SSc onset were(46.0±2.4)years old,of which 16 patients(53.3%)were classified as diffuse cutaneous systemic sclerosis(dcSSc).Correlation analysis showed a positive correlation between men and 5-HT(r=0.520,P=0.003).The native T1 value on cardiac magnetic resonance in SSc patients were(1302.4±46.5)ms,which was negatively correlated with serum 5-HT(r=-0.405,P=0.026).Gender and native T1 value were enrolled in Model 1.Gender,native T1 value,and reflux esophagitis were enrolled in Model 2 for multiple linear regression analysis.Gender of SSc patients and 5-HT in Model 1(β:671.399,95%CI:534.430,808.368,P<0.001),and Model 2(β:670.398,95%CI:534.598,806.199,P<0.001)were significantly correlated.Native T1 value was significantly correlated with 5-HT in model 1(β:-1.510,95%CI:-2.238,-0.782,P<0.001),and Model 2(β:-1.474,95%CI:-2.198,-0.750,P<0.001).Conclusions 5-HT may be related to early myocardial lesions in SSc patients,and the correlation between them needs to be confirmed by further research with expanded sample size.
