清代医家陈士铎与沈菊人治疗翻胃学术思想异同探析

摘　　要：陈士铎和沈菊人同为清代浙江人,学术风格相似,二人均认为“郁”是导致翻胃的重要病因,同时强调胃虚为本,土虚木贼;其次脾虚生湿、湿热为患亦是翻胃的重要病因病机。陈士铎强调脏腑之间的平衡,以“胃为肾之关”阐释翻胃的根本病机,认为肾水亏耗、命门火衰是翻胃的本源,治疗上求之于肾中水火;其次膈上痰血互结,痞塞不通会影响胃主受纳。沈菊人辨证求因、审因论治,注重患者的生活饮食习惯、情志以及外感病对胃功能的影响;胃阳受损,寒饮内积日久,秋间变为翻胃;治疗上以辛苦甘酸通泄为治疗大法。CHEN Shiduo and SHEN Juren were both from Zhejiang during the Qing Dynasty,with similar academic styles.Both believed that“depression”is an important cause of stomach reflux,while emphasizing that stomach deficiency is the foundation and earth deficiency is over-restricted by wood.Secondly,spleen deficiency generating dampness,and damp heat are also important etiology and pathogenesis of stomach reflux.CHEN Shiduo emphasized the balance between the zangfu-organs,and explained the fundamental pathogenesis of stomach reflux with“stomach is the pass of kidney”.He believed that kidney water consumption and fire failure of Mingmen were the origin of stomach reflux,and the treatment was based on water and fire in the kidney.Thirdly,the phlegm and blood on the diaphragm are combined,and the obstruction of the stomach will affect the reception of the stomach.SHEN Juren stressed the influence of patients’living and eating habits,emotions and exogenous diseases on gastric function in determination of etiologic factor and treatment based on syndrome differentiation.Fourthly,the stomach yang is damaged,and the cold fluid retention accumulates for a long time,which turns into stomach reflux in autumn.In the treatment,the main method is to removing heat with herbs acrid,bitter,sweet and sour in flavor.
