会阴侧切对足月初产女性产后盆底结构的近期影响

摘　　要：目的 观察会阴侧切对足月初产女性产后盆底结构的近期影响。方法 回顾性分析于产后6~8周经会阴实时三维盆底超声检查的初产女性,根据有无使用会阴侧切技术分为会阴侧切组及对照组,获取受检者在静息、最大Valsalva及盆底肌收缩状态的数据,统计分析使用会阴侧切技术对初产女性产后盆底结构的近期影响。结果 共有330例女性入组,其中会阴侧切组240例,对照组90例。两组间一般资料比较无统计学意义。静息状态下会阴侧切组及对照组间膀胱颈位置、膀胱位置、子宫位置有统计学意义,直肠壶腹部位置无统计学意义,最大Valsalva状态及盆底肌收缩状态下两组间各项盆底超声测量指标均无统计学意义。结论 应用会阴侧切术对足月初产女性产后盆底结构的近期影响与未使用者对比没有明显差异。Objective To observe the recent effect of lateral episiotomy on the pelvic floor structure in postpartum women with first labor at term.Methods A retrospective analysis was conducted on the primipara women who underwent real-time three-dimensional pelvic floor ultrasonography via perineum in our department at 6-8 weeks after delivery.They were divided into lateral episiotomy group and control group according to whether the delivery lateral episiotomy performed or not.The pelvic floor data in the resting state,the maximum Valsalva state and the pelvic floor muscle contraction state were obtained,and the recent effect of the use of the lateral episiotomy on the pelvic floor structure were statistically analyzed.Results A total of 330 women were enrolled,including 240 in the lateral episiotomy group and 9o in the control group.There was no statistically significant difference in general data between the two groups.There were statistically significant differences in the position of bladder neck,bladder and uterus between the two groups at rest,and there was no statistically significant difference in the position of rectal ampulla.There was no statistical difference in the pelvic floor ultrasonic measurement indexes between the two groups at the maximum Valsalva state and the pelvic floor muscle contraction state.Conclusions There is no significant difference in the recent effect of lateral episiotomy on the pelvic floor structure in postpartum women with first labor at term compared with non-users.
