保产无忧散促进气血两虚型足月妊娠孕妇宫颈成熟的疗效及其机制

摘　　要：目的 观察保产无忧散促进气血两虚证型足月妊娠孕妇宫颈成熟的疗效,并运用网络药理学及分子对接技术进一步探讨相关机制。方法 气血两虚型足月初产妇100例,随机分为对照组与观察组各50例。对照组给予常规母婴监护,观察组在对照组基础上予保产无忧散治疗。观察并记录两组宫颈评分、中医证候积分及母婴结局。通过TCMSP数据库筛选保产无忧散的活性成分及其对应靶点,检索GeneCards、OMIM、TTD、DrugBank、DISGENENT等数据库获得宫颈成熟相关靶点,取两者的交集获得交集靶点;运用STRING数据库构建交集靶点的蛋白质相互作用(PPI)网络,并将交集靶点导入Metascape进行GO、KEGG富集分析;将PPI网络图导入Cytoscape3.9.1软件,以Degree中位数2倍以上为条件筛选核心靶点,在核心靶点中选取Degree前10的活性成分(配体)与疾病靶点(受体),用AutodockTool1.56软件进行分子对接,并用PyMOL2.5软件对结果进行可视化。结果 观察组宫颈评分、顺产成功率高于对照组,中医证候积分低于对照组(P均<0.05)。筛选得到保产无忧散的主要化学成分对应靶点224个、宫颈成熟相关靶点6 474个,二者交集靶点172个。保产无忧散主要通过影响MAPK信号通路、雌激素信号通路、NF-κB信号通路来促进宫颈成熟。保产无忧散促进宫颈成熟的核心靶点主要为TNF、MAPK1、ESR1。结论 保产无忧散可促进气血两虚型足月妊娠孕妇宫颈成熟、提高顺产率,其可能通过作用于TNF、MAPK1、ESR1等靶点,调控MAPK信号通路、雌激素信号通路、NF-κB信号通路等相关通路,从而发挥促进宫颈成熟的作用。Objective To observe the effect of Baochan Wuyou powder on cervical ripening in full-term pregnant women with syndrome of dual deficiency of qi and blood,and further to explore the related mechanism by using network pharmacology and molecular docking technology.Methods Totally 100 full-term primiparas with syndrome of dual deficiency of qi and blood were randomly divided into the control group and observation group,with 50 cases in each group.The control group was given routine maternal and infant monitoring,and the observation group was given Baochan Wuyou powder on the basis of the control group.The cervical score,TCM syndrome score and maternal and infant outcomes of the two groups were observed and recorded.TCMSP database was used to screen the active ingredients and their corresponding targets of Baochan Wuyou powder.The cervical ripening-related targets were obtained by searching GeneCards,OMIM,TTD,DrugBank,etc,and the intersection targets of the two was obtained;the protein-protein interaction(PPI) network of intersection targets was constructed by using STRING database,and the intersection targets were imported into metascape for GO and KEGG enrichment analysis;the PPI network was imported into Cytoscape 3.9.1,and the core targets were screened under the condition of more than twice the median Degree.The active ingredients(ligands) in the top 10 of the Degree value were selected from the core targets and the disease targets(receptors).The molecular docking was carried out by AutodockTool 1.56,and the results were visualized by PyMol2.5.Results The cervical score and the success rate of spontaneous labor in the observation group were higher than those in the control group,and the TCM syndrome score was lower than that in the control group(all P<0.05).The screening results showed that there were 224 corresponding targets and 6 474 targets related to cervical ripening in the main chemical components of Baochan Wuyou powder,with a total of 172 overlapping targets;Baochan Wuyou powder promoted cervical ripenin
