基于临床护理路径的健康教育对门诊糖尿病患者血糖水平及自我管理行为的影响

摘　　要：目的探讨基于临床护理路径(clinical nursing pathway,CNP)的健康教育在门诊糖尿病患者中的应用价值。方法选择2020年7月—2022年7月龙岩市第二医院门诊部收治的糖尿病患者92例为研究对象,按随机数表法分为对照组(n=46)和观察组(n=46)。对照组采用常规健康教育,观察组采用基于CNP的健康教育,两组均护理4周。对比两组血糖水平、自我管理能力、生活质量。结果护理后,观察组空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1c)低于对照组,糖尿病自我管理行为量表(SDSCA)中血糖监测、特殊饮食等维度评分及总分、生活质量调查问卷(QOL-30)评分高于对照组,差异有统计学意义(P<0.05)。结论基于CNP的健康教育能够有效改善门诊糖尿病患者血糖水平及自我管理行为,提升生活质量。Objective To explore the value of health education based on clinical nursing pathway(CNP)in outpatients with diabetes.Methods 92 diabetic patients admitted to the outpatient department of the Second Hospital of Longyan City from July 2020 to July 2022 were selected as the study objects,and were divided into control group(n=46)and observation group(n=46)according to random number table method.The control group received conventional health education,and the observation group received health education based on CNP.Both groups were nursed for 4 weeks.Blood glucose levels,self-management ability and quality of life were compared between the two groups.Results After nursing,fasting blood glucose(FPG),2-hour postprandial blood glucose(2 hPG)and glycosylated hemoglobin(HbA1c)in the observation group were lower than those in the control group,and scores of blood glucose monitoring,special diet and other dimensions and total scores in the Diabetes Self-Management Behavior Scale(SDSCA)and Quality of life questionnaire(QOL-30)in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05).Conclusion Health education based on CNP can effectively improve the blood glucose level and self-management behavior of outpatients with diabetes,and improve the quality of life.
