加减补肺汤对肺气虚型尘肺病患者的临床疗效

摘　　要：目的考察加减补肺汤对肺气虚型尘肺病患者的临床疗效。方法74例患者随机分为对照组和观察组,每组37例,对照组采用常规治疗,观察组在对照组基础上加用加减补肺汤,疗程15 d。检测临床疗效、中医证候评分、肺功能指标(FEV l、FVC、FEV l/FVC)、炎性指标(IL-8、ICAM-1)、氧化应激指标(SOD、GSH-Px、MDA)、肺纤维化指标(HA、LN)、SGRQ评分变化。结果观察组总有效率高于对照组(P<0.05)。治疗后,2组中医证候评分、炎性指标、MDA、肺纤维化指标、SGRQ评分降低(P<0.05),肺功能指标、SOD、GSH-Px升高(P<0.05),以观察组更明显(P<0.05)。结论加减补肺汤可改善肺气虚型尘肺病患者中医证候评分、SGRQ评分,并通过改善通气功能、调节机体氧化或抗氧化功能、抑制肺部炎性反应来延缓疾病进展。AIM To investigate the clinical effects of Modified Bufei Decoction on patients with pneumoconiosis due to Lung Qi Deficiency Pattern.METHODS Seventy-four patients were randomly assigned into control group(37 cases)for 15-day intervention of conventional treatment,and observation group(37 cases)for 15-day intervention of both Modified Bufei Decoction and conventional treatment.The changes in clinical efficacy,TCM syndrome scores,pulmonary function indices(FEV l,FVC,FEV l/FVC),inflammatory indices(IL-8,ICAM-1),oxidative stress indices(SOD,GSH-Px,MDA),pulmonary fibrosis indices(HA,LN)and SGRQ score were detected.RESULTS The observation group demonstrated higher total effective rate than the control group(P<0.05).After the treatment,the two groups displayed decreased TCM syndrome scores,inflammatory indices,MDA,pulmonary fibrosis indices,SGRQ score(P<0.05),and increased pulmonary function indices,SOD,GSH-Px(P<0.05),especially for the observation group(P<0.05).CONCLUSION For the patients with pneumoconiosis due to Lung Qi Deficiency Pattern,Modified Bufei Decoction can improve TCM syndromes scores and SGRQ score,and delay disease progression by the improvement of ventilation functions,regulation of body oxidative or antioxidant functions,and inhibition of pulmonary inflammatory responses.
