早期完全血运重建介入措施对非ST段抬高型心肌梗死合并多支血管病变患者左心室扭转的影响

摘　　要：目的研究早期完全血运重建介入措施对非ST段抬高型心肌梗死(NSTEMI)合并多支血管病变患者左心室扭转的影响。方法选取保定市第一中心医院2018年11月-2020年11月收治的126例NSTEMI合并多支血管病变患者作为研究对象,根据介入措施的不同分为完全血运重建组(CR组)和部分血运重建组(IR组),各63例,CR组采用完全血运重建介入措施,IR组采用部分血运重建介入措施。以二维斑点追踪成像技术分析比较2组的心尖段旋转峰值、基底段旋转峰值、左心室整体净扭转峰值和左心室射血分数(LVEF)。结果术后1周,2组患者心尖段旋转峰值、基底段旋转峰值、左心室整体净扭转峰值和LVEF值较术前均有上升,但差异无统计学意义(P>0.05);术后6个月,2组患者心尖段旋转峰值、基底段旋转峰值、左心室整体净扭转峰值和LVEF值比较,CR组均高于IR组,差异有统计学意义(P<0.05);术后1年,2组患者心尖段旋转峰值、基底段旋转峰值、左心室整体净扭转峰值和LVEF值比较,CR组均显著高于IR组,差异有统计学意义(P<0.05)。结论早期完全血运重建和部分血运重建都可使NSTEMI合并多支血管病变患者的左心室扭转得到改善,但早期完全血运重建的改善效果更为明显。Objective To study the effect of early complete revascularization intervention on left ventricular torsion in patients with non ST segment elevation myocardial infarction(NSTEMI)complicated with multi vessel disease.Methods 126 patients with NSTEMI complicated with multi vessel lesions treated in Baoding first central hospital from November 2018 to November 2020 were selected as the research objects.They were divided into complete revascularization group(CR-group)and partial revascularization group(IR group)according to dfferent interventional measures,with 63 cases in each group.The CR group was treated with complete revascularization intervention,and the IR group was treated with partial revascularization intervention.The apical segment rotation peak,basal segment rotation peak,left ventricular global net torsion peak and left ventricular ejection fraction(LVEF)of the two groups were analyzed and compared by two-dimensional speckle tracking imaging technologyResults One week afer operation,the peak value of apical rotation,the peak value of basal rotation,the peak value of left ventricular global net torsion and LVEF increased in the two groups,but the difference was not statistically significant(P>0.05);Six months after operation,the peak value of apical rotation,the peak value of basal rotation,the peak value of left ventricular global net torsion and LVEF in CR group were significantly higherthan thoseinIR group(P<0.05);Oneyearafteroperation,thepeak value ofapical rotation,thepeak value ofbasal rotation,the peak value oflef ventricular global net torsion and LVEF in CR group were significantly higher than those in IR group(P<0.05).Conclusion Both early complete revascularization and partial revascularization can improve the left ventricular torsion in patients with NSTEMI complicated with multi vessel disease,but the effect of early complete revascularization is more obvious.
