摘　　要：目的观察黑热病患者骨髓象变化。方法选取陇南市第一人民医院1990年7月-2021年9月有完整的骨髓报告单的初发黑热病病例共计482例,对其骨髓象进行分析。结果杜氏利什曼原虫检出率为76.56%,原虫密度“+”者占51.77%;“++”者占15.04%。骨髓增生程度,活跃者占87.34%;粒系增生活跃占64.52%;红系增生活跃占85.68%;淋巴细胞系>20%者占31.33%;浆细胞≥3.0%者占8.09%。单张骨髓涂片巨核细胞>7个者占87.67%。结论黑热病骨髓利什曼原虫检出率较高,是诊断黑热病的重要方法,阳性者原虫密度以“+”“++”者为主。骨髓以增生活跃为主,粒系、红系增生活跃,可伴浆细胞增多,可伴噬血细胞增多,巨核细胞增生活跃。Objective To observe the changes of bone marrow images in patients with Kala-azar.MethodssAtotal of 482 initial cases with complete bone marrow reports from July 1990 to September 2021 in the first people's hospital of Longnan city were selected for bone marrow images analysis.Results The detection rate of Leishmania duringi was 76.56%,and the density of"+"accounted for 51.77%;"++"accounted for 15.04%.The degree of bone marrow hyperplasia,active accounted for 87.34%;Granulogenic hyperplasia accounted for 64.52%;Active erythroid hyperplasia accounted for 85.68%;Lymphocyte line>20%accounted for 31.33%;Plasma cell≥3.0% accounted for 8.09%.The number of megakaryocytes in single bone marrow smear more than 7 accounted for 87.67%.Conclusion The high detection rate of bone marrow leishmaniasis is an important method for diagnosis of leishmaniasis,and the positive protozoa density is mainlyly"+"and"++".Bone marrow is mainly hyperproliferated active,granule and erythroid hyperplasia are active,which can be accompanied by plasmacytosis and hemophagocytosis,megakaryocyte hyperplasia active.
