补阳还五汤治疗腰椎间盘突出症临床应用价值的Meta分析

摘　　要：目的评价补阳还五汤治疗腰椎间盘突出症的临床应用价值。方法应用循证医学研究方法,在CNKI、万方、VIP、PubMed及Cochrane Library数据库中,检索2010年1月1日—2021年4月1日应用补阳还五汤治疗腰椎间盘突出症的临床随机对照试验。根据纳入和排除标准筛选文献、提取资料和偏倚风险评估,对符合纳入标准的研究采用RevMan5.4软件进行数据的Meta分析。结果纳入15篇文献,共1326例患者。分析结果显示:与对照组比较,单用补阳还五汤或常规治疗基础上加服补阳还五汤能明显提高临床疗效(总体有效率RR=1.21,95%CI[1.15,1.28],P<0.00001;优良率RR=1.48,95%CI[1.30,1.67],P<0.00001),降低视觉模拟疼痛(VAS)评分(MD=-1.05,95%CI[-1.29,-0.80],P<0.00001),改善腰椎功能积分(JOA)(MD=3.34,95%CI[2.53,4.15],P<0.00001),差异均有统计学意义。结论补阳还五汤治疗腰椎间盘突出症有确切疗效,可显著提高临床疗效,缓解腰腿疼痛,改善临床症状。Objective To evaluate the clinical application value of Buyang Huanwu Decoction(补阳还五汤)in the treatment of lumbar intervertebral disc herniation.Methods he randomized controlled trials of Buyang Huanwu Decoction in the treatment of lumbar intervertebral disc herniation from January 1,2010 to April 1,2021 were retrieved from CNKI,WanFang,VIP,PubMed and Cochrane Library.According to the inclusion and exclusion criteria,the literature was screened,the data was extractedand the bias risk was assessed.The Meta-analysis on the studies meeting the inclusion criteria were carried out by using RevMan5.4 software.Results A total of 15 literatures including 1326 patients were included.The results showed that:compared with the control group,Buyang Huanwu Decoction alone or on the basis of the conventional treatment could significantly improve the clinical efficacy(overall effective rate RR=1.21,95%CI[1.15,1.28],P<0.00001);excellent and good rate RR=1.48,95%CI[1.30,1.67],P<0.00001),decreased the visual analog pain(VAS)score(MD=-1.05,95%CI[-1.29,-0.80],P<0.00001]and improved the lumbar functional score(JOA)(MD=3.34,95%CI[2.53,4.15],P<0.00001).The differences were statistically significant.Conclusion Buyang Huanwu Decoction has a definite effect on the treatment of lumbar intervertebral disc herniation,and can significantly improve the clinical effect,relieve the pain of waist and legand improve the clinical symptoms.
