运用计量学分析穴位埋线治疗失眠医案用穴规律

摘　　要：目的探讨穴位埋线疗法治疗失眠的穴位运用规律。方法选取穴位埋线治疗失眠文献所载典型医案之临证埋线处方,运用计量学方法进行用穴频数、穴位归经、用穴比率、运用强度、穴均强度等计量学指标分析。结果统计53篇文献,涉及病例1981例,共得53首临证埋线处方,共用穴42个,268穴次;奇穴3穴,3穴次;经外穴1穴,11穴次。穴位数最多为膀胱经,使用穴数最多(10穴),任脉使用5穴次之。膀胱经、脾经运用强度均较高,分别1.0和0.212,运用总频次膀胱经最高(132穴次),脾经(28穴次),任脉(22穴次),小肠经(1)和大肠经(1)最低。用穴比率心包经和心经最高(0.22),其次肝经和任脉均为(0.21)及膀胱经(0.15)、脾经(0.14)相对较高,穴均强度膀胱经最高(0.1),次为脾经(0.071),心包经(0.064),奇穴、小肠经、大肠经最低(0.08);就各经单穴而言,可见以心俞运用频次最高(38),次之为肾俞(30)、脾俞(27)、三阴交(23)、肝俞(22)。结论以方剂计量学方法分析穴位埋线治疗失眠穴位处方,能客观总结诸多医家经验,可为学习经络腧穴理论与穴位埋线临床运用提供有效参考,亦可成为其他文献研究之借鉴。Objective To explore the application rules of acupoint catgut embedding therapy in the treatment of insomnia.Methods The typical prescription of acupoint catgut-embedding in the treatment of insomnia was selected,and the metrological indexes such as the frequency of acupoint use,acupoint meridian return,acupoint use ratio,acupoint use intensity and acupoint average intensity were analyzed by metrological method.Results A total of 53 literatures were collected,involving 1981 cases.A total of 53 clinical embedding prescriptions were obtained,including 268 points with 42 common points.Odd points 3 points,3 points;External points 1,11 points;The most acupoints are bladder meridian,and the most points are used(10 points),followed by Renmai(5 points).The intensity of bladder meridian and spleen meridian were higher,1.0 and 0.212,respectively.The total frequency of bladder meridian was the highest(132 points),spleen meridian(28 points),Renmai(22 points),small intestine meridian(1)and large intestine meridian(1)was the lowest.The ratio of pericardium and heart meridian was the highest(0.22),followed by liver meridian and Ren meridian(0.21),bladder meridian(0.15)and spleen meridian(0.14).The average point intensity of bladder meridian was the highest(0.1),followed by spleen meridian(0.071),pericardium meridian(0.064),and qi point,small intestine meridian and large intestine meridian(0.08).In terms of single points,Xinshu was the most frequently used acupoint(38),followed by Shenshu(30),Pishu(27),Sanyinjiao(23)and Ganshu(22).Conclusion This paper analyzes the acupoint prescription of catgut-embedding in the treatment of insomnia with the method of prescription metrology,can objectively summarize the experience of many doctors,enough to provide an effective reference for learning the theory of meridians and acupoints and clinical application of catgut-embedding in acupoints,and can also be used for reference in other literature research.
