摘 要:目的采用荟萃分析(Meta)方法评价补阳还五汤干预肺纤维化实验动物模型的疗效。方法检索CNKI,Wanfang,VIP,PubMed,Web of Science,EMbase数据库,全面收集公开发表的补阳还五汤干预肺纤维化大鼠/小鼠随机对照试验(randomized controlled trial,RCT)。检索时间从建库到2022年10月。由2名系统评价员按照纳入与排除标准独立进行文献筛选、资料提取和质量评价,采用RevMan 5.3软件进行Meta分析。结果共纳入10篇文献,5个RCTs。Meta分析结果显示:实验组在肺泡炎症积[MD=1.19,95%CI(0.14,2.24),P=0.03]、肺纤维化积分[MD=0.27,95%CI(-1.18,1.37),P<0.00001]、肿瘤坏死因子α(TNF-α)[MD=-5.60,95%CI(-14,2.81),P<0.00001]、转化生长因子β1(TGF-β1)[RR=20.57,95%CI(4.81,5.82),P<0.00001]、Smad蛋白[MD=0.27,95%CI(0.10,0.44),P=0.001],均优于糖皮质激素对照组。结论补阳还五汤可有效减轻动物模型的肺纤维化,与改善肺组织结构,减少TNF-α、TGF-β1、Smad表达相关。Objective A meta-analysis(Meta)method was used to systematically evaluate the efficacy of Buyang Huanwu Decoction(补阳还五汤)in intervening pulmonary fibrosis in animal experiments.Methods Search CNKI,Wanfang,VIP,PubMed,Web of Science,EMbase databases,and comprehensively collect publicly published randomized controlled trials(RCT)of Buyang Huanwu Decoction intervention in pulmonary fibrosis rats/mice.The search time is from the establishment of the database to April 2021.Two systematic reviewers independently carried out literature screening,data extraction and quality evaluation according to the inclusion and exclusion criteria,and used RevMan 5.3 software for meta-analysis,Results A total of 10 articles and 5 RCTs were included.The results of Meta analysis showed that the experimental group was in alveolar inflammation[MD=1.19,95%CI(0.14,2.24),P=0.03],pulmonary fibrosis score[MD=0.27,95%CI(-1.18,1.37),P<0.00001],Tumor Necrosis Factorα(TNF-α)[MD=-5.60,95%CI(-14,2.81),P<0.00001],Transforming Growth Factorβ1(TGF-β1)[RR=20.57,95%CI(4.81,5.82),P<0.00001],Smad protein[MD=0.27,95%CI(0.10,0.44),P=0.001]all had a positive therapeutic effect than the control group.Conclusion Buyang Huanwu Decoction can effectively reduce pulmonary fibrosis in animal model,it is related to reduce the pathological changes of lung tissue and decrease the expression of TNF-α,TGF-β1,Smad.
