活血化瘀汤治疗结核性胸膜炎的应用

摘　　要：目的对结核性胸膜炎患者应用西医治疗和活血化瘀汤治疗的临床效果进行分析探讨。方法选择2019年1月—2022年1月医院收治的80例结核性胸膜炎患者,按照随机分组法把80例患者分为对照组和观察组各40例,对照组患者接受西医治疗,观察组在西医治疗基础上加用活血化瘀汤治疗,观察两组临床效果、中医证候积分、炎症因子水平及胸膜厚度。结果观察组临床有效率明显高于对照组(P<0.05),治疗前,患者气促、发热、咳痰、咯血、咳嗽、胸痛各项中医证候积分、炎症因子水平及胸膜厚度均无较大差异(P>0.05),治疗后,观察组患者气促、发热、咳痰、咯血、咳嗽、胸痛各项中医证候积分明显低于对照组(P<0.05),观察组患者肿瘤坏死因子-a,转化生长因子-β1各项炎症因子水平明显低于对照组(P<0.05),治疗2、8周时,观察组患者胸膜厚度明显低于对照组(P<0.05)。结论结核性胸膜炎患者长时间采取西医治疗容易发生耐药情况,而在西医治疗基础上加用活血化瘀汤治疗可快速改善患者的胸膜增厚,缓解气促、发热、咳痰、咯血、咳嗽、胸痛等临床症状,减轻患者肿瘤坏死因子-a,转化生长因子-β1各项炎性反应,临床效果显著,值得推广。Objective To analyze the clinical effect of western medicine treatment and Huoxue Huayu Decoction(活血化瘀汤)on tuberculous pleurisy patients.Methods A total of 80 patients with tuberculous pleurisy admitted to our hospital from January 2019 to January 2022 were selected,and 80 patients were randomly divided into control group and observation group,with 40 patients in each group.The control group was treated with western medicine,and the observation group was treated with Huoxue Huayu Decoction on the basis of western medicine.Clinical effect,traditional Chinese medicine syndrome score,inflammatory factor level and pleural thickness were observed.Results The clinical effective rate in the observation group was significantly higher than that in the control group(P<0.05).Before treatment,there were no significant differences in traditional Chinese medicine syndrome scores,inflammatory factor levels and pleural thickness of patients with shortness of breath,fever,expectoration,hemoptysis,cough and chest pain(P>0.05).After treatment,The scores of traditional Chinese medicine syndrome of shortness of breath,fever,expectoration,hemoptysis,cough and chest pain in the observation group were significantly lower than those in the control group(P<0.05),and the levels of tumor necrosis factor-A and transforming growth factor-β1 inflammatory factors in the observation group were significantly lower than those in the control group(P<0.05).The pleural thickness in observation group was significantly lower than that in control group(P<0.05).Conclusion Tuberculous pleurisy patients treated with western medicine for a long time are prone to resistance,and on the basis of western medicine combined with promoting blood circulation to remove blood stasis soup treatment can rapidly improve patients with pleural thickening,relieve the shortness of breath,fever,cough,sputum,hemoptysis,cough,chest pain clinical symptoms,reduce the patients with tumor necrosis factor-a,transforming growth factor beta 1-the inflammatory reaction,clin
