摘 要:目的探讨高尿酸血症海勤人员肾结石的患病情况及肾结石发生的影响因素。方法回顾性分析2016年1月至2022年11月入联勤保障部队某康复疗养中心康复疗养的高尿酸血症海勤人员514例,根据泌尿系B超检查是否有肾结石发生,分为肾结石阳性组(n=78)和肾结石阴性组(n=436)。通过统计海勤疗养人员一般资料,计算其体重指数(BMI)和腰臀比;生化实验检测血肌酐、血尿酸,计算尿酸排泄分数。采用单因素和Logistic多因素回归分析,揭示肾结石发病的潜在相关危险因素。结果514例高尿酸血症海勤疗养人员中,肾结石患者78例(15.17%),无肾结石436例(84.83%);高尿酸血症海勤人员肾结石发生率随着年龄(χ_(趋势)^(2)=6.738,P<0.05)和血尿酸(χ_(趋势)^(2)=5.981,P<0.05)水平的提高而相应增加。高尿酸血症海勤疗养人员肾结石阴性组和肾结石阳性组的年龄、吸烟史、BMI、腰臀比、三酰甘油、尿比重、血肌酐、血尿酸、尿酸排泄分数比较差异有统计学意义(P<0.05或P<0.01)。多因素Logistic回归结果显示,在高尿酸血症海勤人员中,年龄≥41岁、BMI≥26 kg/m^(2)、血尿酸≥540μmol/L、血肌酐≥133μmol/L是肾结石发生的潜在相关危险因素(P<0.05或P<0.01)。结论海勤人员特定的环境因素易导致高尿酸血症发生,高尿酸血症海勤人员的年龄、BMI、血尿酸和血肌肝水平是肾结石发生的潜在相关危险因素。Objective To investigate the general status of kidney stones in naval personnel with hyperuricemia and risk factors of kidney stone occurrence.Methods A retrospective analysis was conducted on 514 naval personnel with hyperuricemia who were admitted to a rehabilitation and recuperation center of PLA Joint Logistic Support Force from January 2016 to November 2022.Based on the results of B-ultrasound examination of the urinary system,they were divided into two groups:kidney stone positive group(n=78)and kidney stone negative group(n=436).Demographic data of the research subjects were collected,including body mass index(BMI)and waist-to-hip ratio.Biochemical tests were performed to measure blood creatinine(Cr)and uric acid levels,and uric acid excretion fraction was calculated.Single-factor logistic regression analysis and multiple-factor logistic regression analysis were used to identify the potential risk factors of kidney stone occurrence.Results Among the 514 naval personnel with hyperuricemia,78 cases(15.17%)had kidney stones and 436 cases(84.83%)did not.The incidence of kidney stone in the naval personnel with hyperuricemia increased with age(χ_(Trend)^(2)=6.738,P<0.05)and blood uric acid level(χ_(Trend)^(2)=5.981,P<0.05).There were significant differences between the kidney stone positive group and the kidney stone negative group in terms of age,smoking history,BMI,waist-to-hip ratio,triglyceride,specific gravity of urine,blood Cr,blood uric acid,and uric acid excretion fraction(P<0.05 or P<0.01).Multi-factor logistic regression results showed that age≥41,BMI>26 kg/m^(2),uric acid≥540μmol/L,and Cr level≥133μmol/L were potential risk factors for kidney stone occurrence in the naval personnel with hyperuricemia(P<0.05 or P<0.01).Conclusion Some specific environmental factors can cause hyperuricemia in naval personnel.The potential risk factors for kidney stone occurrence in naval personnel with hyperuricemia are age,BMI,blood uric acid level,and Cr level.
