高原红细胞增多症并发下肢动脉硬化闭塞症综合治疗一例

摘　　要：目的:探讨高原红细胞增多症患者发生下肢动脉硬化闭塞症的发病机制、临床特征及诊疗预后。方法:回顾性分析1例高原红细胞增多症并发下肢动脉硬化闭塞症患者的临床资料。该患者久居高原地区,因右下肢足趾皮肤溃烂2个月在阿坝州藏医医院就诊,根据病史、体征、辅助检查被诊断为高原红细胞增多症并下肢动脉硬化闭塞症,病情复杂危重,若治疗不当有截肢的风险。予以个体化的中西医结合联合藏医药综合治疗方案。结果:治疗后患者的缺氧症状明显改善,右下肢足趾皮肤创面愈合。结论:高原红细胞增多症合并下肢动脉硬化闭塞症是由多种机制的参与导致的。中西医结合联合藏医药的个体化综合治疗方案可有效缓解患者的缺氧症状,遏制并发症,改善患者的生活质量。Objective:To explore the pathogenesis,clinical characteristics and prognosis of lower extremity arteriosclerosis obliterans in high altitude polycythemia patients.Methods:The clinical data of 1 case was retrospectively analyzed.The patient had been living in the plateau area for a long time,and the skin of the toe of the right lower limb was ulcerated for 2 months.According to the medical history,signs and auxiliary examination,the patient was diagnosed as the disease.The condition was complicated and critical,and there was a risk of amputation if the treatment was not proper.The integrative medicine plus Tibetan medicine was given to the patient.Results:After the individualized comprehensive treatment,the patient’s hypoxia symptoms were significantly ameliorated,and the wound of the toe of the right lower limb was healed.Conclusion:High altitude hypercytosis plus lower extremity arteriosclerosis occlusions is caused by multiple mechanisms.The individualized comprehensive treatment plan of the integrative medicine plus Tibetan medicine can effectively relieve hypoxia symptom,curb complications,and improve the quality of life of the patient.
