不同流派手法在脊柱疾病治疗中的应用及机制研究进展

摘　　要：颈椎病的临床发病率较高,由于发病机制复杂、临床疗效差等原因,该病成为骨伤科难治性疾病之一。中医学认为,“筋出槽、骨错缝”是颈椎病的核心病机。随着手法研究的深入,临床上用于治疗颈椎病的整脊手法类型也比较多。手法治疗是通过治疗者的徒手力量或借助某些简单的器械作用于患者身体局部,以防治疾病的一种治疗方法。中医手法又称为按摩或推拿。手法治疗在骨科疾病尤其是筋伤等疾病方面疗效显著,是目前临床最为常用的有效方法之一。中医手法经过多年的传承和发展,已经演化出多种流派,如长安朱氏骨伤流派、清宫正骨流派、岭南林氏正骨推拿流派、陇中正骨学术流派、岳阳张氏骨伤流派等。不同流派的手法虽有差异,但用于治疗骨科疾病皆有效果,不仅可以加强患者颈部的气血循环,还能改善“骨错缝”的结构应力异常状态等。文章整理不同流派关于脊柱疾病(尤其是颈椎病)的学术思想相关文献,对其手法治疗的操作要点和作用机制进行归纳,以期为临床提供借鉴。Cervical spondylosis has a high clinical incidence rate.Due to the complex pathogenesis and poor clinical efficacy,this disease has become one of the intractable orthopedic diseases.TCM believes that the core pathogenesis of cervical spondylosis is muscle digression and bone dislocation.With the deepening of manipulation research,there are many types of chiropractic manipulation used to treat cervical spondylosis clinically.Manipulation therapy is a therapeutic method to prevent and treat diseases by the bare hands strength of the therapist or some simple tools to act on the patient’s body parts.TCM manipulation is also called massage in TCM.Manipulation therapy has a significant effect on orthopedic diseases,especially on injuries of muscles and bones,and is one of the most commonly used effective methods in clinical practice.After years of inheritance and development,TCM manipulation has evolved into many schools,such as Chang’an ZHU’s Bone Trauma School,Qinggong’s Bone Setting School,Lingnan LIN’s Bone Setting Massage School,Longzhong’s Bone Setting Academic School,Yueyang ZHANG’s Bone Setting School,etc..Although there are differences in different manipulation schools,they are all effective in treating orthopedic diseases.They can not only strengthen the blood circulation in the neck of patients,but also ameliorate the abnormal structural stress state of bone dislocation.In this paper,the related literature on the academic thoughts of different manipulation schools in the treatment of spinal diseases(especially cervical spondylosis)are collated to summarize the key operation points and action mechanism of manipulation,with a view to providing a reference for clinical practice.
