温针灸联合中药热敷激痛点治疗腰背肌筋膜疼痛的效果分析

摘　　要：目的:评估温针灸联合中药热敷激痛点治疗腰背肌筋膜疼痛的临床效果。方法:选取2019年3月-2021年6月收治的98例腰背肌筋膜疼痛患者,通过随机数字表法分为常规组(49例)和联合组(49例)。常规组应用中药热敷激痛点治疗,联合组则联合温针灸进行治疗。比较两组临床疗效、不良反应发生情况、腰背功能评分、压痛阈和腰椎活动度。结果:联合组总有效率明显高于常规组(P<0.05)。两组治疗后Oswestry功能障碍指数问卷表(Oswestry Disability Index,ODI)评分均明显降低,相比常规组,联合组ODI评分明显更低;Fugl-Meyer、改良Barthel指数(Modified Barthel Index,MBI)评分均明显提高,相比常规组,联合组明显更高(P<0.05)。治疗后两组压力疼痛耐受阈(Pressure Pain Tolerance Threshold,PPTO)和压力疼痛感觉阈(Pressure Pain Discomfort Threshold,PPDT)均明显提高,相比常规组,联合组明显更高(P<0.05)。治疗后两组腰椎前屈、后伸角度均明显提高,相比常规组,联合组明显更高(P<0.05)。联合组不良反应(腹部不适、精神萎靡、头晕、皮痒各1例)发生率比常规组(腹部不适、精神萎靡、头晕各1例)高,差异无统计学意义(P>0.05)。结论:应用温针灸联合中药热敷激痛点治疗腰背肌筋膜疼痛患者效果更好。此法可明显改善患者腰背功能,提高压痛阈、腰椎活动度,而且安全性较高。Objective:To analyze the clinical effect of warm acupuncture combined with TCM medicine hot compress at myofascial trigger point on lumbar back myofascial pain.Methods:A total of 98 patients with lumbar back myofascial pain from March 2019 to June 2021 were divided into the conventional group(49 patients)and combined group(49 patients)by random numerical method.The conventional group took TCM medicine hot compress at myofascial trigger point treatment,and the combined group was treated by warm acupuncture.The clinical efficacy and adverse effects in the two groups were compared,and the lumbar and back function score,stress-pain threshold,and lumbar activity before and after treatment were compared.Results:The total effective rate in the conventional group was significantly higher than the combined group(P<0.05).The Oswestry disability index(ODI)scores in the combined group was significantly lower than the conventional group.The Fugl-Meyer and the modified Barthel index(MBI)scores were significantly higher,and those in the combined group were significantly higher than the conventional group(P<0.05).After treatment,the pressure pain tolerance threshold(PPTO)and pressure pain discomfort threshold(PPDT)were significantly higher in the combined group than the conventional group(P<0.05).After treatment,the lumbar anterior flexion and posterior extension angles were significantly improved in both groups,which in the conventional group were significantly higher than the combined group(P<0.05).The incidence of adverse reactions in the combined group(Abdominal discomfort,depression,dizziness and itchy skin itching were reported in 1 case respectively)was higher than the conventional group(Abdominal discomfort,depression and dizziness were reported in 1 case respectively),and the difference was not statistically significant(P>0.05).Conclusion:The application of warm acupuncture combined with TCM medicine hot compress at myofascial trigger point on lumbar back myofascial pain shows better effects and higher safety,significan
