活血通脉片在多发性骨髓瘤周围神经病变中的应用

摘　　要：多发性骨髓瘤是一种恶性浆细胞病,患者早期症状往往表现为关节疼痛、骨痛等。周围神经病变是多发性骨髓瘤的一个主要并发症,有大量多发性骨髓瘤患者合并周围神经病变。中医临床研究发现,多发性骨髓瘤以五脏亏虚为本,尤以脾肾亏虚为主,因此脾肾亏虚与气血亏虚紧密相关,加之化疗药物损伤,可损伤脏腑阴阳,加重气血亏虚,肢体关节不利,筋脉失养,呈多发性骨髓瘤周围神经病变的发病基础。多发性骨髓瘤周围神经病变与经络病变密切相关,邪毒侵袭,伏于经络关节,阻碍气血运行,瘀血滋生。瘀毒闭阻经脉导致经脉关节气血输注不利,筋脉关节肌肉机能减退是多发性骨髓瘤周围神经病变的病理概括,瘀、毒为重要病理因素。中成药活血通脉片具有行气活血,通脉止痛的功效,可减轻炎症反应、保护神经组织、改善周围神经病变症状。文章结合案例探讨活血通脉片的方药组成及药理研究,以期为临床治疗提供理论参考。Multiple myeloma is a malignant plasma cell disease,patients with early symptoms often show joint pain,bone pain and so on.Peripheral neuropathy is a major complication of multiple myeloma,a large number of patients with multiple myeloma associated with peripheral neuropathy.TCM clinical research has found that Multiple myeloma is based on the deficiency of the five organs,especially the deficiency of the spleen and kidney,so the deficiency of the spleen and kidney is closely related to the deficiency of Qi(气)and blood,coupled with the damage of chemotherapy drugs,it can damage the internal organs Yin and Yang(阴阳),aggravate the deficiency of Qi and blood,limb and joint are unfavorable,and the muscles and veins are denourished,which is the basis of the pathogenesis of multiple myeloma peripheral neuropathy.Multiple myeloma peripheral neuropathy is closely related to meridian lesions,evil poison invasion,lying in the meridian joints,hindering the flow of Qi and blood,and blood stasis.Obstruction of meridians leads to unfavorable Qi and blood transfusion of meridian joints,and muscular hypofunction of tendons and joints is a pathological summary of peripheral neuropathy of multiple myeloma,and stasis and toxicity are important pathological factors.Chinese patent medicine the Huoxue Tongmai tablet(活血通脉片)has the effect of promoting Qi to activate blood,and relieving pain.This paper through cases discusses the composition and pharmacology of the Huoxue Tongmai tablets,in order to provide a theoretical reference for the treatment of multiple myeloma peripheral neuropathy.
