马小军从气郁论治良性甲状腺结节的经验

摘　　要：甲状腺结节是指在甲状腺内的肿块,临床上将甲状腺结节分为良性和恶性。良性甲状腺结节是内分泌门诊的临床常见病,该病迁延不愈,可在一定程度上影响患者的身心健康。中医药在良性甲状腺结节的治疗方面有独特的优势,不但可以改善患者的不适症状,而且可使部分患者的结节缩小甚至消失,具有疗效较好、不良反应小、安全可靠等优点。甘肃天水市中医医院马小军主任医师根据多年的临床经验,提出良性甲状腺结节多由情志致病,气郁是瘿病的核心病机,气郁日久可导致气郁痰阻、气郁致瘀、气郁化火等病机演变,所以在治疗时应当从气郁入手论治,将理气疏郁,消瘿散结的治疗原则贯穿于整个病程,在根据病机演变辅以理气化痰、活血化瘀、清热泻火之法,以半夏厚朴汤为基础方剂,再根据病机演变随证加减,临床疗效显著。文章结合案例介绍马小军主任治疗良性甲状腺结节的经验,以供临床参考。Thyroid nodule refers to a lump in the thyroid gland.Thyroid nodules are clinically classified into benign and malignant nodules.Benign thyroid nodule is a common clinical disease in endocrine clinics,and the condition persists,which can affect the physical and mental health of patients to a certain extent.TCM medicine has unique advantages in the treatment of benign thyroid nodule.It can not only ameliorate the patient’s discomfort symptoms,but also make the patient’s nodules shrink or even disappear,with good curative effect,smaller adverse reactions,higher safy,and reliability.Based on many years of clinical experience,Chief Physician MA Xiaojun proposes that benign thyroid nodules are mostly caused by emotions,Qi(气)depression is the core pathogenesis of gall,Qi depression can lead to Qi depression phlegm obstruction,Qi depression to stasis,Qi depression and fire and other pathogenic evolution,so in the treatment should start from Qi depression,the treatment principle of rational Qi sooth,eliminate galls and disperse throughout the entire course of the disease,in accordance with the evolution of the disease mechanism supplemented by the method of rational Qi phlegm,blood circulation and stasis,clear heat and ephemeral fire,take the Banxia Houpo decoction(半夏厚朴汤)as the basic formula,and then add and subtract with the evidence according to the evolution of the disease,the clinical effect is remarkable.In this paper,the experience of Director MA Xiaojun in the treatment of benign thyroid nodules is introduced for a clinical reference.
