张兰教授治疗毒性弥漫性甲状腺肿并发情绪障碍的验案举隅

摘　　要：毒性弥漫性甲状腺肿是自身免疫性疾病,此病属于中医“瘿病”范畴,主要累及甲状腺,导致甲状腺肿大和甲状腺机能亢进。常见的临床表现包括低热、食欲亢进、手抖、消瘦、腹泻以及眼睛突出等。本病常因情志刺激引起,因神经系统兴奋引发情绪障碍。辽宁中医药大学附属医院张兰教授指导中医内科瘿病研究与教学多年,认为瘿病和郁证两者关系紧密,郁病可引发瘿病,瘿病会加重郁病,郁病还会导致瘿病迁延不愈。因此,瘿病联合郁病的发作与肝、心、脾密不可分。张兰教授采用桂枝甘草龙骨牡蛎汤联合半夏厚朴汤治疗毒性弥漫性甲状腺肿伴随情绪障碍,取得了显著疗效。文章通过分享验案2例,介绍张教授论治毒性弥漫性甲状腺肿伴情绪障碍的经验,以期为临床提供参考。Toxic diffuse goiter(Graves disease)is an autoimmune disease,which belongs to the category of Ying disease(瘿病)in TCM,and mainly involves the thyroid gland,leading to goiter and hyperthyroidism.Common clinical manifestations include low-grade fever,bulimia,hand shaking,emaciation,diarrhea,and prominent eyes.This disease is often caused by mood disorders.Professor ZHANG Lan from the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine has guided the research and teaching of Ying disease in TCM for many years,believes that Ying disease and depression disease are closely related.Depression disease can cause Ying disease,Ying disease can aggravate the depression disease,depression disease can also lead to Ying disease to delay.Therefore,the occurrence of Ying disease complicated by depression disease is inseparable from the liver,heart and spleen.Professor ZHANG Lan used the Guizhi Gancao Longgu Muli decoction(桂枝甘草龙骨牡蛎汤)and the Banxia Houpo decoction(半夏厚朴汤)to treat toxic diffuse goiter with mood disorder,and achieved a significant curative effect.By sharing two cases,we introduce professor ZHANG’s experience in treating toxic diffuse goiter with mood disorder,in order to provide a clinical reference.
