富马酸替诺福韦二吡呋酯片联合胸腺法新治疗慢性乙型肝炎的效果

摘　　要：目的观察富马酸替诺福韦二吡呋酯片联合胸腺法新治疗慢性乙型肝炎的效果。方法选取2021年1月至2022年9月郑州市第一人民医院收治的98例CHB患者作为研究对象,按照随机数字表法分为单一组和联合组,各49例。单一组接受富马酸替诺福韦二吡呋酯片治疗,联合组在单一组基础上接受胸腺法新治疗。比较两组疗效、载体转阴率[乙型肝炎病毒脱氧核糖核酸(HBV-DNA)转阴率、乙型肝炎e抗原(HBeAg)转阴率]、治疗前后肝功能[天门冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、总胆红素(TBIL)]、肝纤维化指标[Ⅳ型胶原蛋白(C-Ⅳ)、透明质酸(HA)、Ⅲ型前胶原蛋白(PCⅢ)]水平及不良反应发生率。结果单一组和联合组治疗前AST、ALT、TBIL、C-Ⅳ、HA和PCⅢ水平差异无统计学意义(P>0.05)。治疗后,联合组总有效率、HBV-DNA转阴率、HBeAg转阴率高于单一组(P<0.05),AST、ALT、TBIL、C-Ⅳ、HA、PCⅢ水平低于单一组(P<0.05);两组不良反应发生率对比差异无统计学意义(P>0.05)。结论富马酸替诺福韦二吡呋酯片联合胸腺法新治疗CHB效果确切,可促进HBV-DNA、HBeAg转阴,改善患者肝功能和肝纤维化指标,且治疗安全性高。Objective To observe the effect of tenofovir dipifuroxide fumarate tablets combined with thymalfasin in chronic hepatitis B(CHB).Methods A total of 98 CHB patients admitted to Zhengzhou First People’s Hospital from January 2021 to September 2022 were selected as the research subjects,and were divided into single group and combination group using a random number table method,with 49 patients in each group.The single group was treated with tenofovir dipifuroxide fumarate tablets,and the combined group was treated with thymalfasin on the basis of the single group.The efficacy,vector negative conversion rate[hepatitis B virus deoxyribonucleic acid(HBV-DNA)negative conversion rate,hepatitis Be antigen(HBeAg)negative conversion rate],liver function before and after treatment[aspartate aminotransferase(AST),alanine aminotransferase(ALT),total bilirubin(TBIL)],the levels of liver fibrosis indicators[typeⅣcollagen(C-Ⅳ),hyaluronic acid(HA),typeⅢprocollagen(PCⅢ)]and the incidence of adverse reactions were compared.Results There was no statistical difference in levels of AST、ALT、TBIL、C-Ⅳ、HA and PCⅢbetween the single group and combined group.After treatment,the total effective rate,HBV-DNA negative conversion rate,and HBeAg negative conversion rate of the combined group were higher than those of the single group(P<0.05),while the levels of AST,ALT,TBIL,C-Ⅳ,HA,and PCⅢwere lower than those of the single group(P<0.05).There was no statistically difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Tenofovir disoproxil fumarate tablet combined with thymalfasin has a definite effect in the treatment of CHB,which can promote the negative conversion of HBV-DNA and HBeAg,improve patients’liver function and liver fibrosis indicators,and has high treatment safety.
