木丹颗粒联合常规西药治疗糖尿病周围神经病变患者的效果

摘　　要：目的探讨木丹颗粒联合常规西药达格列净、二甲双胍、甲钴胺治疗糖尿病周围神经病变(DPN)的临床效果及对患者神经传导、氧化应激、血液流变学的影响。方法选取医院2020年1月至2022年4月收治的118例DPN患者,通过随机数字表法分为常规西药组和联合组,各59例。常规西药组接受达格列净、二甲双胍、甲钴胺治疗,联合组在常规西药组的基础上接受木丹颗粒联合治疗。治疗2周后,比较两组疗效、血糖指标[糖化血红蛋白(HbA1c)、空腹血糖(FPG)、餐后2 h血糖(2 h PG)]、多伦多临床评分系统(TCSS)、正中神经、腓总神经感觉传导速度(SNCV)、运动传导速度(MNCV)、氧化应激指标[超氧化物歧化酶(SOD)、8-羟基脱氧鸟苷(8-OHdG)、总抗氧化能力(T-AOC)、丙二醛(MDA)]、血液流变学指标[全血高切黏度(HSV)、全血低切黏度(LSV)、血浆黏度(PSV)、血细胞比容(HCT)]、不良反应。结果治疗后,联合组总有效率(96.61%)高于常规西药组(81.36%)(P<0.05);联合组TCSS评分、8-OHdG、MDA、PSV、HSV、LSV、HCT低于常规西药组,正中神经、腓总神经SNCV、MNCV、SOD、T-AOC高于常规西药组(P<0.05);联合组不良反应总发生率(6.78%)与常规西药组(3.39%)比较,差异无统计学意义(P>0.05)。结论木丹颗粒联合常规西药DPN可提升疗效,有效改善患者的血糖指标和神经传导功能,抑制氧化应激,改善血流动力学,优化机体微循环,具有较高的用药安全性。Objective To explore the efficct of Mudan granule combined with conventional western medicine daggligin,metformin,and mecobalamin in the treatment of diabetes peripheral neuropathy(DPN)and its effects on nerve conduction,oxidative stress,and hemorheology of patients.Methods A total of 118 patients with DPN admitted to hospital from January 2020 to April 2022 were randomly divided into conventional western medicine group and combined group,with 59 cases in each group.The conventional western medicine group was treated with daggligin,metformin and mecobalamin,while the combined group was treated with Mudan granule on the basis of the conventional western medicine group.After 2 weeks of treatment,the efficacy,blood glucose indicators[glycosylated hemoglobin(HbA1c),fasting blood glucose(FPG),postprandial 2 h blood glucose(2 h PG)],toronto clinical scoring system(TCSS),median nerve,common peroneal nerve sensory conduction velocity(SNCV),motor conduction velocity(MNCV),oxidative stress indicators[8-hydroxydeoxyguanosine(8-OHdG),superoxide dismutase(SOD),total antioxidant capacity(T-AOC),malondialdehyde(MDA)],hemorheology indexes[whole blood high shear viscosity(HSV),whole blood low shear viscosity(LSV),plasma viscosity(PSV),hematocrit(HCT)],and adverse reactions were compared between conventional western medicine group and Mudan granule combined group.Results After treatment,the total effective rate of the combination group(96.61%)was higher than that of the conventional western medicine group(81.36%)(P<0.05).TCSS score,8-OHdG,MDA,PSV,HSV,LSV,HCT in the combined group were lower than those in the conventional western medicine group,and SNCV,MNCV,SOD,T-AOC in median nerve and common peroneal nerve were higher than those in the conventional western medicine group(P<0.05).The total incidence of adverse reactions had no statistically difference between the Mudan granules combined group(6.78%)and conventional western medicine group(3.39%)(P>0.05).Conclusion The combination of Mudan granule and conventional western medicine
