赞刺治疗急性期耳带状疱疹案

摘　　要：耳带状疱疹多因病毒侵犯面神经、听神经引起,常表现为耳部周围区域出现簇集性小水疱和剧烈神经疼痛症状。耳带状疱疹的临床治疗有一定的难度。此文报告了传统赞刺治疗急性期耳带状疱疹1则,在治疗上采用赞刺、拔罐,辅以抗病毒、止痛、营养神经等常规西医治疗,效佳。患者甲,女,36岁,以右耳周及颈部多发簇集性疱疹6天为主诉就诊。症见:全右耳、右耳周、耳后发际处、颈外侧、颈前多发性簇集性疱疹,自觉耳周灼热伴耳闷感,无耳鸣耳聋,疱疹簇状分布或连成片,各簇疱疹群之间的肤色正常。部分疱疹内含澄清液体,疱疹壁紧张发亮,少数带有脓头,局部红肿。西医诊断:耳部带状疱疹;中医诊断:蛇串疮(肝胆湿热证)。治疗上以清泻肝胆实火,燥湿解毒止痛为治则。采用连续赞刺数处小簇集状疱疹,配以拔罐治疗。行赞刺治疗后,因患处疱疹红肿明显消退,且无新发疱疹,故未再行赞刺拔罐治疗。继而选择患处局部针刺,并配以体针。体针选穴以足厥阴肝经、足少阳胆经为主以清泻肝胆湿热,疏利肝胆气机。辅以抗病毒、止痛、营养神经等常规西医治疗10天后,疱疹干枯结痂,疼痛症状基本消失。1个月后随访,患者未出现面瘫、耳聋等后遗症。Herpes zoster oticus is caused by virus invasion of the facial nerve and auditory nerve,it is often with the manifestation clusters of small blisters and severe nerve pain in the area around the ear.The clinical treatment of herpes zoster otidis has a certain degree of difficulty.This paper reports one case of traditional acupuncture for herpes zoster oticus in the acute phase.It is effective to use repeated shallow puncture and cupping combined with antiviral,analgesic and nutritional nerve therapy.A 36-year-old female patient suffers from multiple clusters of herpes simplex around the right ear and neck for 6 days.The symptoms include multiple clusters of herpes in the right ear,around the right ear,behind the ear hairline,lateral neck and anterior neck.The patient feels burning and tightness around the ear,and there is no tinnitus and deafness.The clusters of herpes are distributed or grouped.The skin color between the clusters of blisters is normal.Some herpes contains clear fluid,tense and shiny,a few are with purulent head and local redness.Western medicine diagnosis of it is herpes zoster oticus.TCM diagnosis of it is snake-like sores(liver-gallbladder dampness-heat syndrome).The treatment principle is to clear the fire of liver-gallbladder,dry dampness,detoxification and pain relief.Several small clusters of herpes cluster are shallowly punctured in a row,which is combined with cupping treatment.After the treatment,the herpes redness and swelling subside significantly and there is no new herpe,so the cupping treatment is not performed again.Then the affected area is selected for local acupuncture,which is combined with the body needle.Acupuncture acupoints are mainly selected along the liver meridian(LR)and the gallbladder meridian(GB)to clear dampness and heat of liver-gallbladder and to relieve Qi(气)of liver and gallbladder.After 10 days of antiviral,analgesic,nutritional and other conventional Western medicine treatment,herpes are dried up and crusted,and the pain symptoms basically disappear.The pat
