血清孕酮、β-hCG联合超声检查诊断早期异位妊娠价值

摘　　要：目的:探讨血清孕酮(P)、β-人绒毛膜促性腺激素(β-hCG)联合超声检查诊断早期异位妊娠价值。方法:选取2020年1月—2023年1月在本院诊治的可疑异位妊娠患者200例临床资料,均经病理学或手术检查,将确诊为异位妊娠患者100例作为观察组,正常宫内妊娠100例作为对照组。均经超声检查,检测血清β-hCG、P水平。受试者工作特征(ROC)曲线分析P、β-hCG水平对异位妊娠的诊断价值。采用四格表分析P、β-hCG联合超声检查对异位妊娠的诊断价值。结果:与对照组相比,观察组血清P、β-hCG水平均降低(P<0.05);血清P、β-hCG诊断异位妊娠的ROC曲线下面积(AUC)分别为0.817、0.869;超声检查诊断异位妊娠阳性率为74.0%;血清P、β-hCG水平联合超声检查诊断异位妊娠的敏感度为98.0%、特异度为77.0%、准确度为87.5%,其诊断效能高于各指标单独诊断。结论:异位妊娠患者血清P、β-hCG水平降低,对异位妊娠有一定诊断价值,联合超声检查可提高异位妊娠诊断效能。Objective:To explore the values of the levels of progesterone and serumβ?human chorionic gonadotropin(β?HCG)combined with the ultrasound examination for diagnosing the early ectopic pregnancy.Methods:The clinical data of 200women with suspected ectopic pregnancy from January 2020to January 2023were selected in this study.Among them,100women with ectopic pregnancy confirmed by pathological examination or surgery were included in the observation group,and 100normal pregnant women were included in the control group.All the women were examined by ultrasound and their serumβ?HCG and progesterone levels were detected.The diagnostic value of the progesterone andβ?HCG levels of the women for their ectopic pregnancy was analyzed by receiver operator characteristic(ROC)curve.The diagnostic value of the progesterone andβ?HCG levels combined with the ultrasound examination of the women for their ectopic pregnancy was analyzed by four grid tables.Results:The serum P andβ?HCG levels of the women in the observation group were significantly lower than those of the women in the control group(P<0.05).The ROC area under curve(AUC)of the serum P level and theβ?HCG level of the women for diagnosing their ectopic pregnancy were 0.817and 0.869.The positive rate of the ultrasound examination of the women for diagnosing their ectopic pregnancy was 74.0%.The sensitivity,the specificity,and the accuracy of the serum P andβ?HCG levels combined with the ultrasonic examination of the women for diagnosing their ectopic pregnancy were 98.0%,77.0%,and 87.5%,respectively.The diagnostic efficacy of the serum P andβ?HCG levels combined with ultrasonic examination was significantly higher than that of the serum P level,theβ?HCG level,and the ultrasonic examination alone.Conclusion:The levels of progesterone and serumβ?HCG of the women with ectopic pregnancy decrease,which has certain diagnostic value for their ectopic pregnancy.The combination of the ultrasound examination and the levels of progesterone and serumβ?HCG of the women can ob
