硒酵母胶囊联合参苓盥洗液对中晚期鼻咽癌患者放化疗期间口腔黏膜炎的影响

摘　　要：目的:探讨硒酵母胶囊联合参苓盥洗液对中晚期鼻咽癌患者放化疗期间口腔黏膜炎的影响。方法:将中晚期鼻咽癌患者117例根据完全随机原则分为试验组(58例)与对照组(59例),对照组予常规西医治疗方案,试验组在此基础上给予硒酵母胶囊联合参苓盥洗液治疗,比较两组患者放化疗期间口腔黏膜炎发病时间、发病率、严重程度、疼痛情况、临床疗效及血清中炎性因子与免疫因子水平。结果:试验组患者放化疗期间口腔黏膜炎发病时间晚于对照组(P<0.05);试验组患者≥3级口腔黏膜炎发病率低于对照组(P<0.05);试验组患者口述分级评分等级低于对照组(P<0.05);试验组、对照组患者近期有效率分别为94.83%(55/58)、89.83%(53/59),差异无统计学意义(P>0.05);治疗后试验组患者血清白细胞介素4、白细胞介素6、C反应蛋白等炎性因子水平及CD4^(+)低于对照组(P<0.05),CD8^(+)、NK等免疫因子水平高于对照组(P<0.05)。结论:硒酵母胶囊联合参苓盥洗液能缓解中晚期鼻咽癌患者放化疗期间口腔黏膜炎临床症状,改善患者生活质量,其机制可能与抑制局部炎性反应、提升免疫功能相关。Objective:To discuss clinical effects of selenium yeast capsule and Shenling irrigating solution in the treatment of oral mucositis in patients with advanced nasopharyngeal carcinoma during radiotherapy and chemotherapy.Methods:All 117 patients were allocated to the experiment group(58 cases)and the control group(59 cases)according to the principles of complete randomization,the control group accepted conventional therapeutic regimen of Western medicine,and the experiment group was treated with selenium yeast capsule and Shenling irrigating solution,to compare the onset time of oral mucositis,the morbidity,severity,pain,clinical effects and the levels of serum inflammatory and immune factors during radiotherapy and chemotherapy between both groups.Results:The experiment group was later than the control group in the onset time of oral mucositis during radiotherapy and chemotherapy(P<0.05);the morbidity of exceeding grade three oral mucositis of the experiment group was lower than that of the control group(P<0.05);VRS of the experiment group was lower than that of the control group(P<0.05);short-term effective rates of both groups were 94.83%(55/58)and 89.83%(53/59)respectively,and the difference had no statistical meaning(P>0.05);after the treatment,the experiment group was lower than the control group in the levels of serum inflammatory factors including IL-4,IL-6,CRP,and CD4^(+)(P<0.05),higher than the control group in the levels of immune factors such as CD8^(+) and NK(P<0.05).Conclusion:Selenium yeast capsule and Shenling irrigating solution in the treatment of oral mucositis in patients with advanced nasopharyngeal carcinoma during radiotherapy and chemotherapy could relieve clinical symptoms and improve the patients’quality of life,and its mechanism might be related to inhibiting local inflammation and raising immune function.
