摘　　要：目的 了解洁净手术室脊柱手术期间无菌灯柄不同时段生物污染情况与影响因素。方法 采用现场采样和细菌定量检测方法,对某医院洁净层流手术室行脊柱手术期间无菌手术灯柄生物污染情况进行调查分析。结果调查期间共使用229个无菌手术灯柄,在手术开始120 min内均未检出细菌;手术持续至180 min时检出8份细菌阳性样本,随着手术持续时间延长,检出阳性数增加。共检出病原菌74株,占前3位的病原菌分别为凝固酶阴性葡萄球菌、大肠埃希菌和微球菌属。多因素分析结果表明,Ⅱ类手术切口、消毒前未用皂液清洁皮肤、灯柄有体液污染和手术间人数> 9是脊柱手术无菌灯柄细菌污染的独立危险因素。结论 随洁净手术室脊柱手术时间延长,细菌污染概率增加,最好在手术进行120 min内更换无菌灯柄。Objective To understand the biological contamination situation and influencing factors of sterile lamp holders at different times during spinal surgery in clean operating rooms.Methods The biological contamination of sterile surgical lamp handle during spinal surgery in a clean laminar flow operating room of a hospital was investigated and analyzed by field sampling and quantitative bacterial detection.Results A total of 229 sterile surgical lamp handles were used during the investigation period,and no bacteria were detected within 120 min after the operation;8 positive samples were detected when the operation lasted to 180 min,and the number of positive samples increased with the extension of the operation duration.A total of 74 strains of pathogenic bacteria were detected,and the top 3 pathogenic bacteria were coagulase-negative Staphylococcus,Escherichia coli and Micrococcus.Multivariate analysis showed that typeⅡsurgical incision,not cleaning skin with soap before disinfection,body fluid contamination on lamp handle,and the number of people in operating room>9 were independent risk factors for biological contamination of sterile lamp handle in spinal surgery.Conclusion With the extension of spinal surgery time in clean operating room,the probability of biological contamination increases.It is better to replace the sterile lamp handle within 120 min after the operation.
