摘　　要：目的 了解某二级甲等综合性医院重症监护室(ICU)医院感染现状,分析感染发生的影响因素,为制订相关干预措施提供科学依据。方法 采取目标性监测方法,对2019—2020年ICU住院患者进行医院感染目标性监测,并对结果进行统计分析。结果 共调查ICU患者1 401例,医院感染发生率为8.28%;感染部位以下呼吸道感染、导尿管相关尿路感染和呼吸机相关性肺炎为主,分别占41.26%、25.17%和9.79%;多因素logistic回归分析显示,女性、有创性置管操作、手术、住院时间≥15 d、合并糖尿病、低蛋白血症和肾功能不全是发生医院感染的独立危险因素。结论 ICU是危重患者集中的区域,医院感染率较高,有创性置管操作是影响较突出的危险因素,应关注重点人群感染预防,加强有创性操作的严格管理。Objective To investigate the status of nosocomial infection in intensive care unit(ICU)of a secondary grade-A general hospital and analyze the influencing factors of infection,so as to provide scientific basis for formulating relevant intervention measures.Methods The targeted monitoring method was adopted to conduct monitoring of nosocomial infection among ICU inpatients from 2019 to 2020,and the results were statistically analyzed.Results A total of 1401 ICU patients were investigated during the study period,and the incidence of nosocomial infection was 8.28%;the main infection sites were lower respiratory tract infection,catheter-associated urinary tract infection and ventilatorassociated pneumonia,accounting for 41.26%,25.17%and 9.79%,respectively.Multivariate logistic regression analysis showed that female,invasive catheterization,surgery,length of stay≥15 d,comorbidity of diabetes,hypoalbuminemia and renal insufficiency were independent risk factors for nosocomial infection.Conclusion ICU is an area where critically ill patients are concentrated,and the rate of nosocomial infection is high.Invasive catheterization is a prominent risk factor.Attention should be paid to the prevention of infection in key populations,and strict management of invasive operations should be strengthened.
