摘  要:目的 评估单把ProGlide联合ExoSeal吻合(Proglide with ExoSeal kissing,PEK)技术对大直径股动脉穿刺口在主动脉介入术后进行主动管理的安全性及有效性。方法 分析2022年6~8月南京医科大学附属苏州医院(苏州市立医院)18例(22个股动脉穿刺口)使用PEK技术进行主动脉介入术患者的临床资料,比较术前及术后1个月股动脉直径及股动脉收缩期峰值流速。结果 患者在术后即刻、24 h及1个月均未发生腹股沟区穿刺相关并发症。股总动脉直径较术前丢失0.22 mm,差异有统计学意义(P<0.001);手术前后股总动脉收缩期峰值流速差异无统计学意义(P=0.402)。结论 PEK技术管理大口径股动脉穿刺点上安全有效,且术后股动脉管腔丢失较少,不存在较长的学习曲线,有利于技术推广。Objective To evaluate the safety and effectiveness of ProGlide with ExoSeal kissing(PEK)technique in the management of large caliber femoral artery puncture site after aortic intervention.Methods The clinical data of 18 patients(22 femoral artery puncture sites)who underwent aortic intervention with PEK technology in Suzhou Hospital Affiliated to Nanjing Medical University(Suzhou Municipal Hospital)from June to August 2022 were analyzed.The diameter and f systolic peak systolic velocity of femoral artery were compared before operation and one month after operation.Results No complications related inguinal puncture occurred immediately,24 hours and 1 month after operation.The diameter of the common femoral artery lost by 0.22 mm compared with that before operation,and the difference was statistically significant(P<0.001).There was no significant difference in the systolic peak flow velocity of common femoral artery before and after surgery(P=0.402).Conclusion PEK technology is safe and effective in the management of large-caliber femoral artery puncture sites,and the loss of femoral artery lumen after operation is minor.There is no long learning curve,which is conducive to the promotion of technology.
