摘 要:目的采用半夏白术天麻汤(Banxia Baizhu Tianma Decoction,BBTD)干预脑缺血再灌注损伤(痰湿加血瘀证)大鼠模型的方法,观察其对缺血性脑卒中的保护作用并初探其对血脂水平的调控作用。方法选取40只SD健康大鼠(SPF级),采用随机法分为4组,具体包括:假手术组、MCAO(痰湿加血瘀证)组(模型组)、MCAO(痰湿加血瘀证)加半夏白术天麻汤组(中药组)、MCAO(痰湿加血瘀证)加阳性药阿托伐他汀组(阿托伐他汀组),每组10只。除假手术组外各组大鼠均造备痰湿加血瘀证模型,并采用线栓法制备脑缺血再灌注模型;假手术组仅暴露血管不插线栓。神经功能缺损评估依据Zea-Longa5分制评分标准;2%氯化三苯四唑啉(TTC)染色观察脑梗死面积;免疫蛋白印迹(Western bolt)法检测各组大鼠脑组织IL-8、p38MAPK、NF-k B蛋白含量;血脂4项试剂盒检测各组大鼠脂代谢水平。结果除假手术组神经功能正常外,其他3组均有不同程度神经损伤。模型组脑梗死面积最大,中药组、阿托伐他汀组面积相对较小。与假手术组相比,造模组大鼠TC、TG、LDL-C升高,HDL-C降低,经药物治疗后TC、TG、LDL-C降低,HDL-C升高。结论半夏白术天麻汤、阿托伐他汀对于减轻大鼠的脑缺血再灌注神经功能缺损症状具有明显效果,可有效缩小脑梗死面积。这种保护作用可能与其降低NFk B、IL-8与p38MAPK蛋白表达水平有关;半夏白术天麻汤能有效改善大鼠血脂水平。Objective To observe the protective effect of Banxia Baizhu Tianma Decoction(BBTD)on ischemic stroke by its intervening the rat model with cerebral ischemia-reperfusion injury(phlegm-dampness complicated with blood stasis syndrome)and explore its regulation effect on the blood lipid level.Methods Forty SD healthy rats(SPF grade)were selected and randomly divided into four groups,including a sham surgery group,an MCAO(phlegmdampness complicated with blood stasis syndrome)group(referred to as the model group),an MCAO(phlegmdampness complicated with blood stasis syndrome)+Banxia Baizhu Tianma Decoction group(referred to as the BBTD group),an MCAO(phlegm-dampness complicated with blood stasis syndrome)+positive drug atorvastatin group(referred to as the atorvastatin group),with 10 rats in each group.except for the sham surgery group,a model of the phlegm-dampness complicated with blood stasis syndrome was established in the rats in the other groups,and a cerebral ischemia-reperfusion model was prepared in them as well,using the thread occlusion method.The sham surgery group only exposed blood vessels without inserting wire plugs.The neurological impairment was evaluated in accordance with the Zea Longa 5-point scoring standard.The volume of cerebral infarction was observed with 2%triphenyltetrazoline chloride(TTC)staining.The protein levels of IL-8,p38MAPK and NF-kB in the brain tissue of rats were detected by Western blot in each group.The lipid metabolism levels were measured with the four blood lipid test kits in the rats of each group.Results except that the sham surgery group had a normal neurological function,all other three groups had varying degrees of neurological damage.The model group had the largest volume of cerebral infarction,while the BBTD group and the atorvastatin group had relatively smaller volumes.Compared with the sham surgery group,the model groups showed an increase in the levels of TC,TG and LDL-C,and a decrease in the level of HDL-C.While after a drug treatment,the levels of TC,TG and LDL-C
