醒脑开窍针刺联合化瘀祛痰汤对急性脑梗死痰瘀互结证患者血小板相关参数、认知功能及神经功能的影响

摘　　要：目的观察醒脑开窍针刺联合化瘀祛痰汤对急性脑梗死痰瘀互结证患者血小板相关参数、认知功能及神经功能的影响。方法选取无锡市中医医院2019年2月-2022年8月收治的100例急性脑梗死痰瘀互结证患者为研究对象,采用单双数标记法分为2组。对照组(n=48)予常规西药治疗,针药结合组(n=52)在对照组基础上予醒脑开窍针刺联合化瘀祛痰汤治疗。结果与治疗前相比,2组Fugl-Meyer评分、日常生活能力(ADL)评分、Mo CA评分治疗后均升高,且针药结合组更高(P<0.05)。2组中医证候积分、NIHSS评分治疗后下降,且针药结合组更低(P<0.05)。与治疗前相比,2组血小板分布宽度(PDW)、平均血小板体积(MPV)、大血小板比率(P-LCR)治疗后下降,且针药结合组更低(P<0.05)。针药结合组不良反应率为13.46%(7/52)与对照组的10.42%(5/48)比较,未见统计学差异(P>0.05)。结论醒脑开窍针刺联合化瘀祛痰汤治疗急性脑梗死痰瘀互结证可抗血小板聚集,改善神经功能,促进认知功能的恢复。Objective To observe the effects of Xingnao Kaiqiao acupuncture combined with Huayu Qutan decoction on the platelet related parameters,cognitive function and neurological function in patients with acute cerebral infarction of the intermingled phlegm and blood stasis syndrome.Methods A total of 100 patients with acute cerebral infarction of the intermingled phlegm and blood stasis syndrome admitted to the Rehabilitation Department of Wuxi Hospital of Traditional Chinese Medicine from February 2019 to August 2022 were selected as the subjects,and were divided into two groups by the single and even number labeling method.The control group(n=48)was treated with conventional western medicine,while the acupuncture and medicine combination group(n=52)was treated with Xingnao Kaiqiao acupuncture combined with Huayu Qutan decoction on the basis of the control group.Results Compared with before treatment,the Fugl-Meyer score,ADL score and MoCA score of the two groups increased after treatment,and the acupuncture and medicine combination group was higher(P<0.05).The TCM syndrome score and NIHSS score of the two groups decreased after treatment,and the acupuncture and medicine combination group was lower(P<0.05).Compared with before treatment,the platelet distribution width(PDW),mean platelet volume(MPV)and large platelet ratio(P-LCR)of the two groups decreased after treatment,and the acupuncture and medicine combination group was lower(P<0.05).There was no statistical difference in the adverse reaction rate(13.46%,7/52 vs 10.42%,5/48)between the two groups(P>0.05).Conclusion Xingnao Kaiqiao acupuncture combined with Huayu Qutan decoction in the treatment of acute cerebral infarction with the syndrome of intermingled phlegm and blood stasis can prevent a platelet aggregation,improve the nerve function and promote the recovery of cognitive function.
