经鼻高流量氧疗治疗一例呼吸衰竭合并严重肺大疱患者的护理体会

摘　　要：患者女,因慢性咳嗽、咳痰、气喘10余年,加重伴呼吸困难7 d。于2022年5月1日就诊,经肺CT及血气分析检查确诊为呼吸衰竭合并严重肺大疱。由于传统治疗效果不佳,故给予经鼻高流量氧疗辅助呼吸治疗,配合基础护理、呼吸功能锻炼、呼吸系统护理、静脉血栓栓塞症预防及心理护理等措施后,患者呼吸困难、咳嗽、咳痰等症状明显好转,遂于5月15日出院。这为呼吸衰竭合并严重肺大疱,且使用无创辅助通气治疗较为困难的患者提供了有益借鉴。The patient is a female who had been suffering from chronic cough,sputum,and asthma for more than 10 years.She presented with worsened symptoms and difficulty breathing for 7 days and was diagnosed with respiratory failure complicated by severe pulmonary bullae after undergoing pulmonary CT and blood gas analysis on May 1,2022.Since traditional treatments were ineffective,she was given high-flow nasal cannula oxygen therapy as an adjunct to respiratory treatment,combined with basic nursing care,respiratory functional exercises,respiratory system care,prevention of venous thromboembolism,and psychological care.The patient's symptoms,including respiratory distress,cough,and sputum,significantly improved.She was discharged on May 15.This provides valuable reference for patients with respiratory failure complicated by severe pulmonary bullae,especially those who find it difficult to undergo non-invasive assisted ventilation treatment.
