摘 要:目的分析丙二醛、对氧磷酶-1在弱精子症患者的含量及与精子质量的关系。方法选择中国中医科学院广安门医院南区自2019年1月至2022年1月接诊的156例弱精子症患者作为观察组,根据弱精子症患者病情严重程度,分为轻、中、重度患者;另选同期的156例男性健康体检者作为对照组。检测所有入选者血清丙二醛、对氧磷酶-1含量,通过受试者工作特征曲线(ROC)下面积(AUC)评价丙二醛联合对氧磷酶-1对弱精子症的诊断效能,使用Pearson相关性分析弱精子症患者血清丙二醛、对氧磷酶-1与前向运动精子率、精子活动率和精子DNA碎片指数的关系。结果观察组轻、中、重度患者血清丙二醛、对氧磷酶-1含量与对照组比较差异均有统计学意义(P<0.05),随着病情严重程度增大,血清丙二醛含量随之升高,对氧磷酶-1含量随之降低;经ROC曲线分析,血清丙二醛联合对氧磷酶-1诊断弱精子症的AUC为0.910,血清丙二醛诊断弱精子症的的AUC为0.665,对氧磷酶-1诊断弱精子症的AUC为0.580;经Pearson相关性分析,弱精子症患者血清精子浓度、前向运动精子率、精子活动率与丙二醛呈负相关(P<0.05),与对氧磷酶-1呈正相关(P<0.05);精子DNA碎片指数与丙二醛呈正相关(P<0.05),与对氧磷酶-1呈负相关(P<0.05)。结论氧化应激指标异常可能与弱精子症发生、发展有关,检测血清丙二醛、对氧磷酶-1有助于反映精子质量,可为弱精子症的诊治提供依据。Objective To analyze the expression of malondialdehyde and paraoxonase-1 in asthenospermia patients and its relationship with sperm quality.Methods A total of 156 asthenospermia patients admitted to the hospital from January 2019 to January 2022 were selected as the observation group.According to the severity of the condition of the patients with asthenozoospermia,the patients were divided into mild patients,moderate patients and severe patients.A total of 156 male healthy subjects were selected as control group.Serum malondialdehyde and paraoxonase-1 expression levels were detected in all selected subjects,and the diagnostic efficacy of malondialdehyde combined with paraoxonase-1 was evaluated by the area under the receiver operating characteristic curve(AUC).Pearson correlation was used to analyze the relationship between serum malondialdehyde,paraoxonase-1 and sperm motility rate,sperm survival rate and sperm DNA fragmentation index in asthenospermia patients.Results The expression levels of serum malondialdehyde and paraoxonase-1 were significantly different among the mild,moderate and severe groups(P<0.05).With the increasing of the severity of disease,the expression level of serum malondialdehyde was increased,and the expression level of paraoxonase-1 was decreased.ROC curve analysis showed that the AUC of serum malondialdehyde combined with paraoxonase-1 in the diagnosis of asthenospermia was 0.910,which was significantly higher than that of malondialdehyde(0.665)and paraoxonase-1(0.580).Pearson correlation analysis showed that sperm motility rate,sperm motility rate and sperm survival rate in asthenospermia patients were negatively correlated with malondialdehyde(P<0.05),and it positively correlated with paraoxonase-1(P<0.05).Sperm DNA fragmentation index was positively correlated with malondialdehyde(P<0.05),and it negatively correlated with paraoxonase-1(P<0.05).Conclusion Abnormal oxidative stress index may be related to the occurrence and development of asthenospermia,detection of serum malondialdehyd
