依据Beers、STOPP和Strand分类法探讨社区医院老年多重用药患者的用药问题

摘　　要：目的采用不同标准探讨医院多重用药老年患者潜在不适当用药(PIM)和药物相关问题(DRPs)的发生率和具体问题。方法采用简便抽样法抽取2018年9月~2019年11月在北京市顺义区空港医院门诊就诊的100例年龄≥65岁的多重用药患者,进行问卷调研,利用2019版Beers标准、2014版STOPP标准、Strand分类系统的DRPs评估方法分别对社区老年患者多重用药问题进行评估。结果100例老年多重用药患者中,根据2019版Beers标准审核出PIM发生率为32%,根据STOPP标准审核出PIM发生率为36%,DRPs的发生率为57%。结论本研究发现,采用不同评估方法评估老年人多重用药人群的PIM与DRPs的发生率不同,本研究采用3种方法全面评估社区老年人多重用药存在的问题,为后续干预老年人多重用药问题提供方向。Objective To explore the incidence of potentially inappropriate medications(PIM)and drug-related problems(DRPs)in older patients with polypharmacy based on different criteria.Methods A simple sampling method was used to select 100 patients aged≥65 years with polypharmacy who were hospitalized in Shunyi Airport Hospital from September 2018 to November 2019 for questionnaire survey.PIMs were identified by the 2019 Beers criteria and the 2014 Screening Tool of Older Persons'Prescriptions(STOPP)criteria,and DRPs were identified by the Strand classification system.Results In 100 older patients with polypharmacy,the incidence of PIMs was 32% according to the 2019 Beers criteria,and 36% according to the STOPP criteria,respectively.The incidence of DRPs was 57%.Conclusion The incidence of PIMs and DRPs varies with different criteria in older patients with polypharmacy.This study provides ideas for the management of polypharmacy in older patients.
