新型加温毯预防高龄前列腺电切手术低体温及应激反应的效果

摘　　要：目的探讨新型肢体加温毯预防高龄前列腺电切手术患者低体温及应激反应的效果。方法选取江苏省徐州市中心医院2017年1月~2020年6月收治的80例前列腺电切手术的高龄患者作为研究对象,按照随机数字表法分为研究组与对照组各40例,对照组采用常规保暖方法,研究组采用新型肢体加温毯保暖方法,比较两组术中体温、血压波动情况,比较术中寒战及躁动发生情况,比较麻醉苏醒情况及围术期应激反应指标(血糖、血乳酸值)的变化。结果研究组入室时、麻醉诱导后即刻、手术开始后30 min、手术结束后即刻、术后30 min、送回病房前的鼻咽部温度及血压值波动小于对照组(P<0.05);术后苏醒情况优于对照组(P<0.05),术中寒战及躁动发生率均低于对照组(P<0.05);入室时、手术开始后30 min、手术结束后及患者苏醒后的血糖、血乳酸波动小于对照组(P<0.05)。结论新型加温毯保温护理对高龄前列腺电切手术患者围术期低体温及应激反应的预防效果较好,值得在临床推广应用。Objective To study the effects of a new limb heating blanket on preventing hypothermia and stress response in advanced aged patients undergoing transurethral resection of the prostate(TURP).Methods Eighty advanced aged patients undergoing TURP in Xuzhou Central Hospital from January 2017 to June 2020 were selected in this study.The patients were divided into a research group(40 cases)and a control group(40 cases)according to the random number table.The patients in the control group were warmed with conventional method,and the patients in the research group were warmed with a new limb heating blanket.The following indicators were compared between the two groups,including the intraoperative body temperature,blood pressure,intraoperative chills and agitation,anesthesia recovery,and perioperative stress response indices(blood sugar,blood lactic acid).Results The fluctuations of nasopharyngeal temperature and blood pressure were less in the research group than in the control group after entering the operation room,immediately after anesthesia induction,30 min from the start of the surgery,the end of surgery,30 min after surgery,and before return to the ward(P<0.05).The postoperative anesthesia recovery was better in the research group than in the control group(P<0.05),and the incidences of intraoperative chills and agitation were lower than in the control group(P<0.05).The blood glucose and blood lactic acid fluctuations were less in the research group than in the control group after entering the operation room,at 30 min from the start of the surgery,the end of surgery,and after anesthesia recovery(P<0.05).Conclusion The new heating blanket care has a good preventive effects on the perioperative hypothermia and stress reaction in the advanced aged patients undergoing TURP.
