慢性肾功能衰竭的腹膜透析患者短期预后不良的影响因素分析

摘　　要：目的分析可能导致慢性肾功能衰竭(CRF)的腹膜透析(PD)患者短期预后不良的影响因素,以期指导该类患者短期预后不良的预防与干预。方法收集本院2016年12月至2017年10月接受PD治疗的90例CRF患者的临床资料,纳入患者的完整资料(包括治疗、随访资料等),记录患者随访3个月的预后情况,将随访期间死亡、转血液透析或联合血液透析等情况列入预后不良;设计基线资料填写表,研究所需的基线资料,初步比较预后不良、预后良好CRF患者的基线资料后,将可能的影响因素纳入单因素及多因素分析,探讨可能导致CRF的PD患者短期预后不良的影响因素。结果治疗结束后,随访3个月内16例患者预后不良,占17.78%;单因素及多因素logistic回归分析结果显示,糖尿病、腹膜炎、透析不充分、白蛋白(Alb)低表达、白介素-6(IL-6)过表达均是CRF的PD患者短期预后不良的影响因素(OR>1,均P<0.05)。结论CRF的PD患者短期预后不良可能受患者伴糖尿病、腹膜炎、透析不充分、Alb低表达、IL-6过表达影响。Objective To analyze the risk factors of poor short-term prognosis in patients with chronic renal failure(CRF)and peritoneal dialysis(PD),to guide the prevention and intervention of poor short-term prognosis of CRF with PD patients in the future.Methods The data of 90 CRF patients who received PD treatment in the hospital from December 2016 to October 2017 were retrospectively analyzed and collected,the data of the included patients were complete(including treatment data,follow-up data,etc.),the prognosis of patients after 3 months of follow-up were recorded,conditions such as death,conversion to hemodialysis or combined hemodialysis were included as the poor prognosis.The baseline data filling table was designed to study the required baseline data.After preliminary comparison of the baseline data of CRF patients with poor prognosis and good prognosis,the possible influencing factors were included in univariate and multivariate analysis to explore the influencing factors that may lead to poor short-term prognosis of PD patients with CRF.Results Among 90 CRF patients within 3 months of follow-up after the end PD treatment,16 had poor prognosis,accounting for 17.78%;after a preliminary comparison of the baseline data of patients with poor prognosis and good prognosis,the data with statistically significant differences were included,after single regression analysis,all the data were included and a multiple regression model was established,the results showed that diabetes mellitus,peritonitis,inadequate dialysis,low expression of albumin(Alb)and over expression of interleukin-6(IL-6)were the influencing factors of poor short-term prognosis in CRF with PD patients(OR>1,all P<0.05).Conclusions The short-term poor prognosis of CRF PD patients may be affected by diabetes mellitus,peritonitis,inadequate dialysis,low expression of Alb and over expression of IL-6.
