基于肌骨超声技术探析针刺得气机理

摘　　要：目的:运用肌骨超声技术探索针刺足阳明胃经部分腧穴得气过程中的解剖特征及得气感评价,研究针刺得气的机理及规律,为经络与腧穴实质的研究提供科学的客观指标。方法:选取健康志愿者7例,针刺穴位依次为足阳明胃经的足三里(右)、上巨虚(右)、条口(右)、丰隆(右)、下巨虚(右)、梁丘(右),共6个腧穴,在健康志愿者得气过程中记录得气感量化评价表,并通过肌骨超声技术动态观察针尖所到的解剖结构,比较足阳明胃经不同腧穴在得气过程中的解剖特征及得气感评价的区别和相关性。结果:肌骨超声图像显示,当针尖刺入小腿骨间膜层时,得气人数较多,得气感评分值最高,得气感最好,此时针尖距皮肤的距离为(2.82±0.50)cm,针尖距胫骨距离为(0.69±0.45)cm;当针尖刺入股骨前结缔组织层时,得气人数较多,得气感评分值最高,得气感最好,此时针尖距皮肤距离为(2.68±0.99)cm,针尖距股骨距离为(0.63±0.32)cm。结论:经络与腧穴存在一定的未知通路,较为可能是结缔组织相关解剖结构,而针刺治病的机理可能是由于某种特殊“能量流”通过经络通道进行传导,使针刺者产生相应得气感,从而起到运行气血、协调阴阳、治病防病的作用。Objective:To explore the anatomic characteristics and the evaluation of obtaining qi in the process of acupuncture at some points of the stomach meridian of foot yang brightness so as to provide a scientific objective index for the study of meridians and the es⁃sence of acupuncture points.Methods:A total of 7 healthy volunteers were selected,and acupuncture points were Zusanli(ST 36,right),Shangjuxu(ST 37,right),Tiaokou(ST 38,right),Fenglong(ST 40,right),Xiajuxu(ST 39,right),Liangqiu(ST 34,right),a total of 6 acupoints.The quantitative evaluation table of qi sensation was recorded in the process of obtaining qi of the healthy volun⁃teers,and the anatomical structure reached by the needle tips was dynamically observed by musculoskeletal ultrasound technology.The anatomic characteristics of different acupoints of the stomach meridian of foot yang brightness in the process of obtaining qi and the difference and correlation of the evaluation of obtaining qi sensation were compared.Results:The musculoskeletal ultrasound images showed that when the needle tip penetrated into the lower leg interosseous membrane,there were more patients with qi arrival,the score of qi sensation was the highest,and the qi sensation was the best.The distance between the needle tip and skin was(2.82±0.50)cm,and the distance between the needle tip and tibia was(0.69±0.45)cm.When the needle tip penetrated into the pre⁃bone connective tissue layer,there were more patients with qi arrival,the score value of qi sensation was the highest,and the qi sensation was the best.At this time,the distance between the needle tip and skin was(2.68±0.99)cm,and the distance between the needle tip and femur was(0.63±0.32)cm.Conclusion:There are some unknown channels between meridians and acupoints,which are more likely connec⁃tive tissue⁃related anatomical structures,and the mechanism of acupuncture treatment may be due to some special"energy flow"con⁃ducting through meridians and collaterals,which makes acupuncture patients have corresponding
