海口市女性盆底功能障碍性疾病的流行病学调查研究

摘　　要：目的调查海口市20~81岁女性盆底功能障碍性疾病(PFD)发病情况、海口地区女性对PFD的认知情况,分析发生PFD影响相关因素,获得海口地区女性PFD的流行病学资料,总结海口地区女性PFD的流行病学特点及相关危险因素,为PFD的发生预防及诊治提供数据支持。方法选取海南省中医院、海口市妇幼保健院、海南妇产科医院、海口市第四人民医院门诊就诊的患者1600例为调查对象。年龄20~81岁,平均(37.40±14.63)岁。通过问卷获得临床资料信息,包括年龄、体质量指数(BMI)、生育情况及盆腔手术史等情况。调查表发放1600份,回收1600份。问卷资料包括一般情况、尿失禁生活质量问卷、盆底功能障碍性疾病问卷、性生活资料问卷、盆底功能追踪表及体格检查。对收集资料进行统计分析。结果1600份调查表中,分娩年龄20~44岁,平均(35.0±2.1)岁。无性生活史79例,有性生活无生育史235例,有生育史1286例,其中阴道分娩982例,剖宫产304例。产次0~4次,平均(2.03±1.10)次。超重336例,占21.00%;肥胖434例,占27.13%;标准595例,占37.19%;低于标准指数235例,占14.69%。单因素分析显示:患者生育年龄越高、分娩次数越多、BMI越大、过低及有子宫全切史的PFD发生率越高,均为危险因素(P<0.05);绝经是独立危险因素(P<0.05)。盆腔脏器脱垂996例,压力性尿失禁1099例,慢性盆腔痛35例,性交障碍15例。41~50岁年龄段POP、SUI发病率较高。Logistic风险回归分析显示:产次多(≥2次)、年龄高、阴道分娩次数越多、子宫全切史及绝经均为PFD及SUI发生的独立危险因素(P<0.05)。结论海口市41~50岁女性PFD患病率高,年龄≥37岁,阴道分娩2次以上,子宫切除术,BMI≥25 kg/m^(2)及绝经均为PFD及SUI发生的独立危险因素。为了预防、降低PFD发病率,需加强对海口市所有女性进行盆底相关知识健康科普教育及已婚已产妇女的筛查和科普教育,更好的改善Objective To investigate the prevalence of pelvic floor dysfunction disease(PFD)among women aged 20~81 years in Haikou City and the cognition of PDF among women in Haikou area,analyze the related factors affecting the occurrence of PDF,obtain the epidemiological data of female PFD in Haikou area,and summarize the epidemiological characteristics and related risk factors of female PFD in Haikou area.To provide data support for the prevention,diagnosis and treatment of PFD.Methods A total of 1600 outpatient patients from Hainan Provincial Hospital of Traditional Chinese Medicine,Haikou Maternal and Child Health Hospital,Hainan Obstetrics and Gynecology Hospital and Haikou Fourth People's Hospital were selected as the investigation objects.Their ages ranged from 20 to 81 years,with an average age of(37.40±14.63)years.Clinical data including age,body mass index(BMI),fertility and pelvic surgery history were obtained through questionnaires.1600 copies of the questionnaire were issued and 1600 copies were recovered.The questionnaire data included general information,urinary incontinence quality of life questionnaire,pelvic floor dysfunction questionnaire,sexual life data questionnaire,pelvic floor function tracking table and physical examination.The collected data were analyzed statistically.Results Among 1600 questionnaires,the age of delivery ranged from 20 to 44 years,with an average age of(35.0±2.1)years.There were 79 cases with no sexual life history,235 cases with no reproductive history,1286 cases with reproductive history,including 982 cases of vaginal delivery and 304 cases of cesarean section.The average number of births was(2.03±1.10)from 0 to 4 times.336 cases(21.00%)were overweight.434 cases(27.13%)were obese.Standard 595 cases,accounting for 37.19%;Below the standard index 235 cases,accounting for 14.69%.Univariate analysis showed that the higher the reproductive age,the more times of delivery,the larger and lower BMI and the higher the incidence of PFD with a history of total hysterectomy(P<0.05).Menop
