宫腔镜联合左炔诺孕酮宫内缓释系统治疗子宫内膜息肉的效果观察

摘　　要：目的研究宫腔镜联合左炔诺孕酮宫内缓释系统治疗子宫内膜息肉的效果。方法选取82例子宫内膜息肉患者,根据随机数字表法分为对照组(40例)与研究组(42例)。对照组予以宫腔镜电切术治疗,研究组予以宫腔镜联合左炔诺孕酮宫内缓释系统治疗。比较两组治疗前后子宫内膜厚度、月经量以及血红蛋白水平,并随访记录患者的疾病复发情况。结果两组患者治疗前子宫内膜厚度比较,差异无统计学意义(P>0.05);研究组患者治疗3个月、6个月后子宫内膜厚度均薄于对照组(P<0.05)。两组治疗前月经失血图(PBAC)评分比较,差异无统计学意义(P>0.05);研究组治疗3个月、6个月后的PBAC评分均低于对照组(P<0.05)。两组治疗前血红蛋白水平比较,差异无统计学意义(P>0.05);研究组治疗3个月、6个月后的血红蛋白水平均高于对照组(P<0.05)。研究组术后6个月随访结束时疾病复发率为4.76%,低于对照组的20.00%(P<0.05)。结论宫腔镜联合左炔诺孕酮宫内缓释系统治疗子宫内膜息肉具有显著疗效,可减少子宫内膜厚度,促进月经恢复,减少疾病复发,值得临床推广。Objective To investigate the effect of hysteroscopy combined with levonorgestrel intrauterine sustained release system in the treatment of endometrial polyps.Methods 82 patients with endometrial polyps were selected and divided into control group(40 cases)and study group(42 cases)according to random number table method.The control group was treated with hysteroscopic electrotomy,and the study group was treated with hysteroscopic combined levonorgestrel intrauterine sustained release system.Endometrial thickness,menstrual volume and hemoglobin levels before and after treatment were compared between the two groups,and disease recurrence was recorded during follow-up.Results There was no significant difference in endometrial thickness between the two groups before treatment(P>0.05).The endometrial thickness of the study group was thinner than that of the control group after 3 and 6 months of treatment(P<0.05).There was no significant difference in PBAC score between the two groups before treatment(P>0.05).The PBAC score of the study group at 3 months and 6 months after treatment was lower than that of the control group(P<0.05).There was no significant difference in hemoglobin level between the two groups before treatment(P>0.05).The hemoglobin level of the study group at 3 months and 6 months after treatment was higher than that of the control group(P<0.05).The recurrence rate of the study group was 4.76%at the end of 6 months follow-up,which was lower than that of the control group 20.00%(P<0.05).Conclusion The hysteroscopy combined levonorgestrel intrauterine sustained release system has a significant effect on the treatment of endometrial polyps,which can reduce endometrial thickness,promote menstrual recovery and reduce disease recurrence,and is worthy of clinical promotion.
