摘  要:目的探讨基于磁共振成像(MRI)检查女性垂体高度与垂体-性腺轴激素水平的关系。方法回顾性分析145例行垂体MRI检查的女性患者的临床资料。利用MRI工作站观察各个序列垂体影像、测量垂体高径、分析垂体高度与性激素水平的关系。结果以垂体上缘形态分为隆起型、平坦型、凹陷型;以垂体柄位置分为垂体柄左偏、右偏、居中。其中垂体形态以平坦型多见,占总病例49.7%,其次为凹陷型(26.9%)、隆起型(23.4%);垂体柄以居中多见,占总病例81.4%,其次为左偏(11.7%)、右偏(6.9%)。女性垂体高度与年龄呈负相关(P<0.05),随着年龄增大,垂体高度呈减低趋势。垂体高度与泌乳素(PRL)、睾酮(T)、孕酮(P)、促黄体生成素(LH)水平呈正相关(P<0.05),与卵泡刺激素(FSH)、雌二醇(E2)水平无相关性(P>0.05)。结论垂体MRI是临床内分泌疾病早发现、早治疗的重要辅助检查方法。女性随着年龄增长垂体高度减低,相关性激素水平呈降低趋势。MRI示垂体高度可以间接评价女性性激素水平的情况,为女性内分泌疾病提供诊断依据和病因支持。Objective To investigate the relationship between pituitary height and pituitary-gonadal axis hormone levels in women based on magnetic resonance imaging(MRI).Methods The clinical data of 145 female patients undergoing pituitary MRI were retrospectively analyzed.MRI workstation was used to observe pituitary images of each sequence,measure pituitary height diameter,and analyze the relationship between pituitary height and sex hormone levels.Results The morphology of pituitary upper margin can be divided into uplift type,flat type and concave type.According to the position of pituitary stalk,it can be divided into left pituitary stalk,right pituitary stalk and center.The morphology of pituitary gland was flat type,accounting for 49.7%of the total cases,followed by concave type(26.9%)and eminence type(23.4%).The pituitary stalk was in the middle,accounting for 81.4%of the total cases,followed by left bias(11.7%)and right bias(6.9%).The height of pituitary gland was negatively correlated with age(P<0.05),and decreased with age.Pituitary height was positively correlated with the levels of prolactin(PRL),testosterone(T),progesterone(P)and luteinizing hormone(LH)(P<0.05),but had no correlation with the levels of follicle stimulating hormone(FSH)and estradiol(E2)(P>0.05).Conclusion Pituitary MRI is an important auxiliary examination method for early detection and treatment of clinical endocrine diseases.The height of pituitary decreased with age and the level of related sex hormones decreased.MRI shows that the pituitary height can indirectly evaluate the level of female sex hormones,providing diagnostic basis and etiological support for female endocrine diseases.
