腹腔镜子宫肌瘤剔除术患者实施快速康复护理的效果评价

摘　　要：目的比较腹腔镜子宫肌瘤剔除术患者分别实施常规护理和快速康复护理效果差异,探讨快速康复护理的临床价值。方法选取96例行腹腔镜子宫肌瘤剔除术患者,根据随机数字表法分为常规护理组与快速康复组,各48例。常规护理组采用常规护理,快速康复组采用快速康复护理。比较两组心理状态、手术指标、术后疼痛程度及术后并发症发生情况。结果护理前,两组焦虑自评量表(SAS)、抑郁自评量表(SDS)评分比较,差异无统计学意义(P>0.05);护理后,两组SAS评分、SDS评分均低于护理前,且快速康复组SAS评分(46.10±4.60)分、SDS评分(45.21±4.87)分均低于常规护理组的(48.22±5.06)分、(48.34±4.95)分(P<0.05)。快速康复组手术时间、术后肛门排气时间、伤口愈合时间均短于常规护理组,术中出血量少于常规护理组,术后24 h的视觉模拟评分法(VAS)评分低于常规护理组(P<0.05)。快速康复组并发症发生率20.83%低于常规护理组的6.25%(P<0.05)。结论在围术期给予腹腔镜下子宫肌瘤剔除术患者快速康复护理,可以缩短住院时间,加快患者康复,值得临床推广。Objective To compare the effects of routine nursing and rapid rehabilitation nursing in patients undergoing laparoscopic myomectomy,and to discuss the clinical value of rapid rehabilitation nursing.Methods 96 patients undergoing laparoscopic myomectomy were selected and divided into routine nursing group and rapid rehabilitation group according to random number table method,48 cases in each group.The conventional nursing group adopted conventional nursing,and the rapid rehabilitation group adopted rapid rehabilitation nursing.The psychological state,surgical indexes,postoperative pain and postoperative complications were compared between the two groups.Results Before nursing,there was no significant difference in self-rating anxiety scale(SAS)and self-rating depression scale(SDS)scores between the two groups(P>0.05).After nursing,SAS score and SDS score of the two groups were lower than before nursing,and SAS score(46.10±4.60)points and SDS score(45.21±4.87)points of the rapid rehabilitation group were lower than those of the conventional nursing group(48.22±5.06)points and(48.34±4.95)points(P<0.05).The operation time,postoperative anal exhaust time and wound healing time of the rapid rehabilitation group were shorter than those of the conventional nursing group,the intraoperative blood loss was less than that of the conventional nursing group,and the visual analog scale(VAS)score 24 h after the operation was lower than that of the conventional nursing group(P<0.05).The complication rate of rapid rehabilitation group was 20.83%lower than that of conventional nursing group(6.25%)(P<0.05).Conclusion In the perioperative period,rapid rehabilitation nursing of patients undergoing laparoscopic myomectomy can shorten the length of hospital stay and accelerate the recovery of patients,which is worthy of clinical promotion.
