前列疏通汤联合非那雄胺治疗良性前列腺增生的临床疗效及对前列腺特异性抗原的影响

摘　　要：目的分析前列疏通汤联合非那雄胺治疗良性前列腺增生(BPH)的临床疗效及对前列腺特异性抗原(PSA)的影响。方法以2021年3月-2022年3月天津市宝坻区人民医院泌尿外科收治的80例BPH患者为研究对象,按照随机数字表法分为对照组(40例)与观察组(40例)。对照组给予非那雄胺治疗,观察组在此基础上联合前列疏通汤治疗,比较两组临床疗效、前列腺体积、血清PSA水平、尿动力学水平[排尿总量、最大尿流率(MFR)、平均尿流率(AFR)]、国际前列腺症状评分(I-PSS)、生活质量量表评分(QOL)及不良反应。结果观察组治疗总有效率高于对照组(P<0.05);两组治疗后前列腺体积、PSA水平小于治疗前,且观察组前列腺体积、PSA水平小于对照组(P<0.05);两组治疗后排尿总量、MFR、AFR均大于治疗前,且观察组排尿总量、MFR、AFR均大于对照组(P<0.05);两组治疗后I-PSS评分低于治疗前,QOL评分高于治疗前,且观察组I-PSS评分低于对照组,QOL评分高于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论前列疏通汤联合非那雄胺在BPH治疗中疗效确切,可缩小患者前列腺体积,降低其血清PSA水平,改善尿流动力学水平,促使症状缓解,提高生活质量,用药安全性良好。Objective To analyze the clinical efficacy of Qianlie Shutong decoction combined with finasteride in the treatment of benign prostatic hyperplasia(BPH)and its effect on prostate specific antigen(PSA).Methods A total of 80 patients with BPH admitted to the Department of Urology,Tianjin Baodi District People's Hospital from March 2021 to March 2022 were divided into control group(40 patients)and observation group(40 patients)according to the random number table method.The control group was treated with finasteride,and the observation group was treated with Qianlie Shutong decoction on this basis.The clinical efficacy,prostate volume,serum PSA level,urodynamic level[total urination,maximum urinary flow rate(MFR),average urinary flow rate(AFR)],international prostate symptom score(I-PSS),quality of life scale score(QOL)and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group(P<0.05).The prostate volume and PSA level of the two groups after treatment were less than those before treatment,and the prostate volume and PSA level of the observation group were less than those of the control group(P<0.05).After treatment,the total amount of urination,MFR and AFR in the two groups were higher than those before treatment,and the total amount of urination,MFR and AFR in the observation group were higher than those in the control group(P<0.05).After treatment,the I-PSS score of the two groups was lower than that before treatment,and the QOL score was higher than that before treatment,while the I-PSS score of the observation group was lower than that of the control group,and the QOL score was higher than that of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Qianlie Shutong decoction combined with finasteride is effective in the treatment of BPH,which can reduce the volume of prostate,reduce the level of serum PSA,improve the level of u
