摘  要:目的:探讨能谱CT单能量技术联合基于模型的自适应统计迭代重建(ASIR-V)算法在胸部双低CT增强扫描中的应用价值。方法:选取60例行胸部CT增强扫描的患者,随机分为能谱A组、B组和对照组各20例。对照组采用传统扫描模式+滤波反投影(FBP)重建,对比剂剂量1.0 mL/kg体质量;能谱组采用能谱扫描模式+ASIR-V,对比剂剂量0.8 mL/kg体质量,其中能谱A组采用70 keV单能量+后置60%ASIR-V进行重建,能谱B组采用前置40%ASIR-V及70 keV单能量联合后置60%ASIR-V进行重建。对比3组对比剂碘用量、客观图像质量(SNR和CNR)、图像主观评分及辐射剂量。结果:对照组对比剂碘用量明显高于能谱组(P<0.05);能谱B组有效剂量(ED)为(3.22±0.69)mSv,较能谱A组及对照组均明显降低(均P<0.05)。3组组间动脉期及静脉期CT值、CNR及SNR比较,除静脉期CT值外,能谱组均明显高于对照组(均P<0.05)。3组图像主观评分差异无统计学意义(P>0.05)。结论:在胸部CT增强扫描中采用前置40%ASIR-V扫描及70 keV单能量联合后置60%ASIR-V能够在降低对比剂碘用量、降低患者辐射剂量的基础上,保证图像质量,满足临床诊断需要。Objective:To explore the application value of spectral CT monochromatic imaging technology combined with a multimodel adaptive statistical iterative reconstruction-V(ASIR-V)algorithm in double-low enhanced CT scans.Methods:Sixty patients with chest enhanced CT scans were enrolled and randomly divided into 2 spectral groups and 1 control group,with 20 cases in each group.In the control group,the traditional scanning mode with the filtering back projection(FBP)algorithm and contrast agent dose of 1.0 mL/kg were used.The spectral imaging mode with ASIR-V and contrast agent dose of 0.8 mL/kg were used in the spectral groups,in which 70 keV monochromatic images combined with post-60%ASIR-V were selected in spectral group A,and 70 keV monochromatic images combined with pre-40%and post-60%ASIR-V were used in spectral group B.The iodine dosage,SNR,CNR,subjective scores and radiation dose were compared in the three groups.Results:The iodine dosage in the control group was obviously higher than that in the spectral groups(P<0.05).The effective dose(ED)of spectral group B was(3.22±0.69)mSv,which was significantly lower than that of the spectral group A and the contral group(both P<0.05).The CT value,CNR and SNR of the three groups in the arterial and venous phases were compared,except for the CT value in the venous phase,the parameters were significantly higher in the spectral groups than those in the control group(all P<0.05).There was no statistically significant difference in the subjective scores of images among the three groups(P>0.05),and all images satisfied the demands of clinical diagnosis.Conclusion:In the chest enhanced CT scan,spectral 70 keV monochromatic images with pre-40%and post-60%ASIR-V can reduce the contrast agent dose and radiation dose without degrading the imaging quality.
