床旁超声无创性评估新型冠状病毒感染中型患者肺部病变的临床研究

摘　　要：目的:探讨床旁超声在无创性评估新型冠状病毒(新冠病毒)感染中型患者肺部病变中的临床意义。方法:选取新冠病毒感染中型患者26例为研究对象,均行肺部超声和CT检查,观察肺部病变、进行影像学评分,并以胸部CT为金标准,分析超声无创性评估新冠病毒感染中型患者肺部病变与CT的一致性。结果:26例患者肺部超声表现:胸膜线连续但不光滑24例,B线增多19例,3例B线呈融合征(<3 mm),同时A线消失,肺小实变征1例,无肺炎表现2例。肺部病变主要分布于胸膜下、肺外周带。超声评估与CT具有较好的一致性(K=0.607,P<0.001)。超声肺部病变评分与胸部CT评分呈显著相关(r=0.862,P<0.001)。结论:床旁超声可无创新评估新冠病毒感染中型患者的肺部病变,具有重要临床意义,值得进一步推广。Objective:To explore the value of noninvasive assessment of lung lesions in patients with medium-type coronavirus disease-19(COVID-19)by bedside ultrasound.Methods:Twenty-six patients with medium-type COVID-19 were selected as the study subjects.All cases were examined by lung ultrasound and CT.The ultrasound and imaging findings of lung lesions were observed and the ultrasound and imaging scores were obtained.CT results were used as the gold standard to evaluate the clinical value of ultrasound in the noninvasive assessment of lung lesions.Results:Lung ultrasound signs in patients mainly manifested that 24 patients had continuous but not smooth pleural lines,19 patients had increased B-lines,3 patients had B-line fusion signs(<3 mm)and A-line disappearance,1 patient had small solid symptoms of the lungs,and 2 patients had no pneumonia.The lung lesions were mainly distributed in the subpleural and peripheral pulmonary zones.There was a good consistency between bedside ultrasound and CT for lung lesions in COVID-19 patients(K=0.607,P<0.001).There was a good correlation between the lung ultrasound score and CT score in evaluating the severity of lung injury(r=0.862,P<0.001).Conclusion:Bedside ultrasound has an important clinical significance for the noninvasive assessment of lung lesions in patients with medium-type COVID-19 and deserves further promotion.
