摘 要:目的观察CT用于精准划分流行季节儿童肺炎支原体(MP)感染类型的价值。方法纳入111例MP感染患儿,记录其主要临床表现.根据CT所见将MP分为支气管炎型(Ⅰ型)、支气管肺炎型(Ⅱ型)、肺段肺炎型(Ⅲ型)、大叶性肺炎型(Ⅳ型)及肺不张型(Ⅴ型),每型包括A、B、C 3个亚型;以ⅡB~C、ⅢB~C、ⅣA~C、ⅤB~C型为高危型,比较高危与非高危MP患儿临床表现的差异。结果111例MP主要临床表现包括发热102例(102/111,91.89%)、咳嗽110例(110/111,99.10%);CT分型包括Ⅰ型23例(23/111,20.72%)、Ⅱ型50例(50/111,45.05%)、Ⅲ型15例(15/111,13.51%)、Ⅳ型15例(15/111,13.51%)、Ⅴ型8例(8/111,7.21%);高危患儿54例(高危组)、非高危患儿57例(非高危组),组间临床表现差异均无统计学意义(P均>0.05)。结论CT可明确显示MP肺部病变程度,进而对流行季节MP感染进行精准影像学分型。Objective To observe the value of CT for classify types of mycoplasma pulmonary(MP)infection in children during epidemic season.Methods Totally 111 children with MP infection were enrolled,and the main clinical manifestations were recorded.According to CT findings,children with MP were classified as bronchitis type(typeⅠ),bronchopneumonia type(typeⅡ),pulmonary segment pneumonia type(typeⅢ),lobar pneumonia type(typeⅣ)and atelectasis type(typeⅤ),each type was furtherly divided into A,B and C subtype.ⅡB—C,ⅢB—C,ⅣA—C andⅤB—C were judged as high-risk MP types.The clinical manifestations of high-risk types were compared with those of non-high-risk MP.Results Fever(102/111,91.89%)and cough(110/111,99.10%)were the main clinical manifestations of MP infection of children.CT classified typeⅠin 23 cases(23/111,20.72%),typeⅡin 50(50/111,45.05%),typeⅢ15 and typeⅣeach in 15(15/111,13.51%)and typeⅤin 8 cases(8/111,7.21%).Fifty-four cases were found with high-risk(high risk group),while non-high-risk group included 57 cases,and no significant difference of clinical manifestations was found between groups(all P>0.05).Conclusion CT could be used for displaying degree of lung invasion and accurate image typing of MP infection in children during epidemic season.
