摘  要:目的探讨老年慢性阻塞性肺疾病(COPD)急性加重期患者血清同源异形盒基因1(Six1)、微小RNA-448-5p(miR-448-5p)表达水平及其临床意义。方法将2021年3月到2022年3月本院住院部收治的老年COPD急性加重期患者110例(COPD急性加重组)、COPD稳定期患者60例(COPD稳定组)纳入研究。另外,选取同期于该院体检的健康者60例作为对照组。收集受试者基本资料。COPD急性加重期患者血清Six1、miR-448-5p水平与临床指标间的相关性采用Pearson法分析;多因素Logistic回归分析影响COPD患者急性加重的可能因素;miR-448-5p、Six1表达与生存资料采用Kaplan-Meier生存曲线进行分析;多因素Cox回归分析影响COPD急性加重患者预后的因素。结果COPD急性加重组、稳定组及对照组Six1水平依次降低(P<0.05),miR-448-5p水平依次显著升高(P<0.05);相关性分析显示,COPD急性加重期患者血清miR-448-5p、Six1水平均与氧分压(PaO_(2))、第1秒用力呼气容积与用力肺活量的比值(FEV_(1)/FVC)、第1秒用力呼气容积占预计值百分比(FEV_(1)%pred)、年龄、二氧化碳分压(PaCO_(2))呈显著相关(P<0.05);多因素Logistic回归分析显示,miR-448-5p、年龄、Six1是COPD发生急性加重的影响因素(P<0.05);死亡组miR-448-5p水平显著低于存活组,Six1水平显著高于存活组(P<0.05);Kaplan-Meier结果显示miR-448-5p高表达患者30 d生存率(80.00%)高于miR-582-5p低表达患者(56.36%),Six1高表达患者30 d生存率(47.27%)低于Six1低表达患者(89.09%);miR-448-5p、Six1是COPD急性加重患者死亡的影响因素(P<0.05)。结论miR-448-5p水平在老年COPD急性加重期患者血清中显著下调、Six1在老年COPD急性加重期患者血清中显著上调,检测血清miR-448-5p、Six1的表达有利于对老年COPD患者病情的评估。Objective To investigate the expression levels of serum homeobox gene 1(Six1)and microRNA-448-5p(miR-448-5p)in elderly patients with acute exacerbation of chronic obstructive pulmonary disease(COPD)and their clinical significance.Methods From March 2021 to March 2022,110 elderly patients with acute exacerbation of COPD,60 patients with stable COPD and 60 healthy physical examination patients were admitted to the inpatient department of our hospital.Basic data of subjects were collected.The correlation between serum Six1 and miR-448-5p levels and clinical indexes in patients with acute exacerbation of COPD was analyzed by Pearson method.Multivariate Logistic regression analysis of possible factors affecting COPD patients with acute exacerbation;The expression and survival data of miR-448-5p and Six1 were analyzed using Kaplan-Meier survival curve.Multivariate Cox regression analysis of factors affecting prognosis of patients with acute exacerbation of COPD.Results Six1 was significantly decreased(P<0.05),and miR-448-5p was significantly increased(P<0.05)among COPD acute plus recombinant,stable and control groups.Correlation analysis showed that serum miR-448-5p and Six1 levels were significantly correlated with PaO_(2),FEV_(1)/FVC,FEV_(1)%Pred,age and PaCO_(2) in patients with acute exacerbation of COPD(P<0.05).Multiple Logistic regression analysis showed that miR-448-5p,age and Six1 were the influencing factors of COPD exacerbation(P<0.05).The level of miR-448-5p in death group was significantly lower than that in survival group,and the level of Six1 was significantly higher than that in survival group(P<0.05).Kaplan-Meier results showed that the 30 d survival rate of patients with high miR-448-5p expression(80.00%)was higher than that of patients with low miR-582-5p expression(56.36%),and the 30 d survival rate of patients with high Mir-448-5p expression(47.27%)was lower than that of patients with low Six1 expression(89.09%).miR-448-5p and Six1 were the influencing factors of death in patients with acute exacerb
