皮瓣修复美容整形外伤所致鼻缺损临床效果及护理研究

摘　　要：目的探讨皮瓣修复美容整形因外伤所致鼻缺损的临床效果,以及系统化的护理对患者心理及整形效果的影响。方法方便选取2019年1月至2021年4月某三甲医院收治的116例外伤所致鼻缺损患者为研究对象,所有患者均接受皮瓣修复鼻美容整形手术,随机将其分为对照组和观察组,每组58例,对照组术后给予常规护理,观察组在常规护理基础上给予系统化皮瓣管理及健康教育指导。采用鼻整形手术效果评估量表(ROE)、温哥华瘢痕评估量表(VSS)、焦虑自评量表(SAS)、抑郁自评量表(SDS)评价两组患者护理前及术后6个月心理状态、瘢痕形成情况及鼻美容整形效果。结果术后6个月,观察组ROE评分为(89.37±9.45)分,高于对照组(73.85±10.37)分,差异有统计学意义(t=8.425,P<0.001);观察组VSS评分为(6.59±1.27)分,低于对照组(8.11±1.65)分,差异有统计学意义(t=5.559,P<0.001)。护理前,两组患者SAS、SDS评分比较差异无统计学意义(P>0.05),术后6个月,观察组患者SAS、SDS评分明显低于对照组,组间及护理前后组内比较,均有统计学意义(P<0.05)。结论皮瓣修复美容整形术是治疗外伤所致鼻缺损的有效方法,能够有效提高缺损鼻功能和外观;系统化的皮瓣管理及健康教育指导能够缓解患者焦虑、抑郁等负性情绪,降低鼻部瘢痕程度,提升鼻整形美容效果,值得临床推广应用。Objective To explore the clinical effects of skin flaps to repair nasal defects caused by trauma,and the influence of systematic nursing on patients'psychology and plastic effects.Methods It is convenient to select 116 patients with nasal defects caused by injuries who were admitted to a tertiary hospital from January 2019 to April 2021 as the research objects.All patients underwent skin flap repair rhinoplasty and plastic surgery,and they were randomly divided into control group and observation group,58 cases in each group,the control group was given routine care after surgery,and the observation group was given systematic flap management and health education guidance on the basis of routine care.Rhinoplasty Surgery Effectiveness Evaluation Scale(ROE),Vancouver Scar Evaluation Scale(VSS),Anxiety Self-Rating Scale(SAS),Depression Self-Rating Scale(SDS)were used to evaluate the two groups of patients before and 6 months after care Mental status,scar formation and the effect of nose cosmetic surgery.Results 6 months after the operation,the ROE score of the observation group was(89.37±9.45)points,which was higher than the control group(73.85±10.37)points,the difference was statistically significant(t=8.425,P<0.001);The observation group's VSS score was(6.59±1.27)points,which was lower than the control group(8.11±1.65)points,and the difference was statistically significant(t=5.559,P<0.001).Before nursing,there was no statistically significant difference in SAS and SDS scores between the two groups of patients(P>0.05).At 6 months after surgery,the SAS and SDS scores of the observation group were significantly lower than those of the control group.Comparisons between groups and before and after nursing.All were statistically significant(P<0.05).Conclusion Skin flap repair and cosmetic surgery is an effective method to treat nasal defects caused by trauma,which can effectively improve the function and appearance of the defect;systematic flap management and health education guidance can relieve patients’anxiety,dep
